Anticipated will directives in adult and elderly patients in palliative care and quality of life assessment: an integrative review

Resumo: | Introduction: Palliative care corresponds to a type of therapeutic approach considering the individual as a whole and providing an improvement in the quality of life for him and for all those involved to minimize physical, psychological, social, and spiritual suffering. The Advance Directives of Will allow the individual to choose, still lucid and oriented, which care they wish to receive in their final moments of life. Objective: To analyze the effects of Advance Directives of Will in patients with terminal illness, to evaluate the quality of life and to identify the level of knowledge of the patient and of the professionals involved. Methodology: literature review, in 3 databases: Pubmed, Medline and Scielo using the health descriptors: “Advance Directives” AND “Palliative Care” AND “Aged”, “Quality of Life” AND “Advance Directives” and “Quality of Life” AND “Advance Directives” AND “Palliative Care”. The collection was carried out in October 2021 and the inclusion criteria were articles available in Portuguese, English or Spanish in national and international journals. The selected period was productions from the last 10 years (2011-2021). Results: For an understanding of the purpose of palliative care, communication is presented as essential, strengthening the links between the team, family members and patients. On the other hand, the lack of communication tends to generate distrust and insecurity in patients and their families. Randomized studies in developed countries that evaluated Advance Guidelines and quality of life found no differences in the quality of life of patients in the control arm and the intervention arm. |

Las Directivas Anticipadas de Voluntad permiten al individuo elegir, aún lúcido y orientado, qué cuidados desea recibir en sus últimos momentos de vida. Objetivo: Analizar los efectos de las Voluntades Anticipadas de Voluntad en pacientes con enfermedad terminal, evaluar la calidad de vida e identificar el nivel de conocimiento del paciente y de los profesionales implicados. Metodología: revisión de literatura, en 3 bases de datos: Pubmed, Medline y Scielo utilizando los descriptores de salud: “Advance Directives” AND “Palliative Care” AND “Aged”, “Quality of Life” AND “Advance Directives” y “Quality of Life” AND “Advance Directives” AND “Palliative Care”. La recolección se realizó en octubre de 2021 y los criterios de inclusión fueron artículos disponibles en portugués, inglés o español en revistas nacionales e internacionales. El período seleccionado fueron producciones de los últimos 10 años (2011-2021). Resultados: Para la comprensión de la finalidad de los cuidados paliativos, la comunicación se presenta como fundamental, fortaleciendo los vínculos entre el equipo, los familiares y los pacientes. Por otro lado, la falta de comunicación tiende a generar desconfianza e inseguridad en los pacientes y sus familias. Los estudios aleatorizados en países desarrollados que evaluaron las Directrices Avanzadas y la calidad de vida no encontraron diferencias en la calidad de vida de los pacientes en el brazo de control y el brazo de intervención.Introdução: Os cuidados paliativos correspondem a um tipo de abordagem terapêutica considerando o indivíduo em sua integralidade e proporcionando melhora na qualidade de vida dele e de todos os envolvidos a fim de minimizar o sofrimento físico, psicológico, social e espiritual. As Diretivas Antecipadas de Vontade permitem ao indivíduo escolher, ainda lúcido e orientado, quais os cuidados desejam receber em seus momentos finais de vida. Objetivo: Analisar os efeitos das Diretivas Antecipadas de Vontade em pacientes com doença terminal, avaliar a qualidade de vida e identificar o nível de conhecimento do paciente e dos profissionais envolvidos. Metodologia: revisão da literatura, em 3 bases de dados: Pubmed, Medline e Scielo usando os descritores em saúde: “Advance Directives” AND “Palliative Care” AND “Aged”, “Quality of Life” AND “Advance Directives” e “Quality of Life” AND “Advance Directives” AND “Palliative Care”. A coleta foi realizada no mês de outubro de 2021 e os critérios de inclusão foram artigos disponíveis nos idiomas português, inglês ou espanhol em periódicos nacionais e internacionais. Resultados: Para uma compreensão da finalidade dos cuidados paliativos a comunicação se apresenta como essencial fortalecendo os elos entre a equipe, os familiares e pacientes. Por outro lado, a falta de comunicação tende a gerar desconfiança e insegurança nos pacientes e seus familiares.
