Protein-calorie adequacy of enteral nutritional therapy and factors associated with waste in intensive care unit patients

Resumo: Patients admitted to the Intensive Care Unit (ICU) have a prevalence of malnutrition higher than 35% at the time of admission, and even critically ill patients with a nutritional diagnosis of eutrophy, after hospital admission, can quickly develop protein-calorie malnutrition. Given the changes and unforeseen events that occur in the ICU with the patient, influencing the decrease in caloric-protein supply and the correct administration of the enteral diet in the ICU, as well as the need to reduce costs and waste, the objective of this research is to evaluate caloric-protein adequacy between closed system Enteral Nutrition Therapy (ENT) prescribed and administered/infused in critically ill patients admitted to an ICU and to verify the main reasons for wastage of enteral diets. This is a longitudinal, observational, and descriptive research, carried out at the University Hospital, in the city of Lagarto (HUL), for 30 days during the year 2021. Data collection was carried out in the ICU, which was selected for presenting the largest number of patients using closed system ENT, targeting adults and elderly in critical condition. Most patients were hospitalized for infectious diseases (29.4%), and the main reason for the indication of ENT was neurological reasons (64.7%). Regarding the nutritional profile of patients, it was observed that 52.9% were underweight. Among the reasons for the wastage of enteral diets, hemodynamic instability, and infusion pump error were cited, but for the most part, professionals had no information about the cause.

Ante los cambios e imprevistos que se producen en la UCI con el paciente, influyendo en la disminución del aporte calórico-proteico y en la correcta administración de dieta enteral en la UCI, así como en la necesidad de reducir costes y residuos, el objetivo de esta investigación es evaluar la adecuación calórico-proteica entre la Terapia Nutricional Enteral (TNE) de sistema cerrado prescrita y administrada/infundida en pacientes críticos hospitalizados en una UCI y verificar los principales motivos del desperdicio.  dietas enterales. Se trata de una investigación longitudinal, observacional y descriptiva, realizada en el Hospital Universitario, en el municipio de Lagarto (HUL), durante un periodo de 30 días durante el año 2021. Se realizó una recolección de datos en la UCI, la cual fue seleccionada porque presentó el mayor número de pacientes que utilizaron TNE de sistema cerrado, dirigidos a adultos y ancianos en estado crítico. La mayoría de los pacientes fueron hospitalizados por enfermedades infecciosas (29,4%), y la razón principal de la indicación de TNE fueron razones neurológicas (64,7%). En cuanto al perfil nutricional de los pacientes, se observó que el 52,9% tenía bajo peso. Entre las razones para desperdiciar dietas enterales se encontraban la inestabilidad hemodinámica y el error de la bomba de infusión, pero la mayoría de los profesionales no tenían información sobre la causa.Os pacientes internados em Unidade de Terapia Intensiva (UTI) apresentam prevalência de desnutrição superior a 35% já no momento da admissão e, mesmo os pacientes críticos apresentando diagnóstico nutricional de eutrofia, após admissão hospitalar, podem desenvolver desnutrição calórica-proteica rapidamente. Tendo em vista as alterações e imprevistos que ocorrem na UTI com o paciente, influenciando na diminuição da oferta calórica-proteica e na correta administração da dieta enteral na UTI, bem como a necessidade de redução de custos e desperdícios, o objetivo dessa pesquisa é avaliar a adequação calórica-proteica entre Terapia Nutricional Enteral (TNE) de sistema fechado prescrita e administrada/infundida em pacientes críticos internados em uma UTI e verificar os principais motivos de desperdício das dietas enterais. Trata-se de uma pesquisa longitudinal, observacional e descritiva, realizada no Hospital Universitário, no município de Lagarto (HUL), durante um período de 30 dias durante o ano 2021. Foi realizado um levantamento de dados na UTI, que foi selecionada por apresentar o maior número de pacientes em uso de TNE de sistema fechado, tendo como público-alvo adultos e idosos em estado crítico. A maior parte dos pacientes foi internada por doenças infecciosas (29,4%), sendo que a principal razão para a indicação de TNE foram motivos neurológicos (64,7%). Em relação ao perfil nutricional dos pacientes, observou-se que 52,9% apresentaram baixo peso.
