Local anesthetics in Dentistry: a literature review

Resumo: Local anesthesia is essential for performing the vast majority of dental procedures. Mortality cases due to local anesthesia are rare. However, cases of morbidity are more common but not always reported. Given the above, this study aimed to review the literature on local anesthetics used in dentistry, highlighting necessary information regarding the choice and amount of anesthetic, preventing the occurrence of complications. For the construction of this work, a bibliographic survey was carried out in the SciVerse Scopus, Scientific Electronic Library Online (Scielo), U.S. National Library of Medicine (PUBMED) and ScienceDirect databases, using the Mendeley reference manager. Scientific studies available in the literature report. Standardizing a solution for general use does not always satisfy the clinical needs and well-being of patients, and can even cause serious problems for them. Although all local anesthetics currently used have similar mechanisms of action, they may differ in terms of duration of action, pharmacological effects, metabolism, associated vasoconstrictor, adverse reactions and, therefore, indications. Thus, the professional must choose the anesthetic solution according to the patient and the procedure to be performed. The best treatment for systemic complications arising from local anesthesia is undoubtedly prevention, through a good anamnesis and physical examination, choosing the safest anesthetic salt for each specific case.

Los casos de mortalidad por anestesia local son raros. Sin embargo, los casos de morbilidad son más comunes, pero no siempre se informan. Teniendo en cuenta lo anterior, este estudio tuvo como objetivo revisar la literatura sobre los anestésicos locales utilizados en odontología, destacando las informaciones necesarias sobre la elección y la cantidad de anestésico, previniendo la aparición de complicaciones. Para la construcción de este trabajo se realizó un levantamiento bibliográfico en las bases de datos SciVerse Scopus, Scientific Electronic Library Online (Scielo), U.S. National Library of Medicine (PUBMED) y ScienceDirect, utilizando el gestor de referencias Mendeley. Informe de estudios científicos disponibles en la literatura. Estandarizar una solución para uso general no siempre satisface las necesidades clínicas y el bienestar de los pacientes, e incluso puede causarles serios problemas. Aunque todos los anestésicos locales utilizados en la actualidad tienen mecanismos de acción similares, pueden diferir en cuanto a duración de acción, efectos farmacológicos, metabolismo, vasoconstrictor asociado, reacciones adversas y, por tanto, indicaciones. Así, el profesional debe elegir la solución anestésica de acuerdo con el paciente y el procedimiento a realizar. El mejor tratamiento de las complicaciones sistémicas derivadas de la anestesia local es sin duda la prevención, mediante una buena anamnesis y exploración física, eligiendo la sal anestésica más segura para cada caso concreto.A anestesia local é essencial para a realização da grande maioria de procedimentos em odontologia. Casos de mortalidade devido à anestesia local são raros. No entanto, casos de morbidade são mais comuns, mas nem sempre são relatados. Diante do exposto, esse estudo objetivou revisar a literatura acerca dos anestésicos locais utilizados na odontologia, destacando informações necessárias quanto à escolha e quantidade de anestésica, prevenindo a ocorrência de complicações. Para a construção deste trabalho foi feito um levantamento bibliográfico nas bases de dados SciVerse Scopus, Scientific Eletronic Library Online (Scielo), U.S. National Library of Medicine (PUBMED) e ScienceDirect, utilizando o gerenciador de referências Mendeley. Estudos científicos disponíveis na literatura relatam. A padronização de uma solução para uso geral nem sempre satisfaz às necessidades clínicas e o bem-estar dos pacientes, podendo até mesmo trazer sérios problemas a eles. Apesar de todos os anestésicos locais utilizados atualmente terem mecanismos de ação semelhantes, eles podem diferir quanto à duração de ação, efeitos farmacológicos, metabolismo, vasoconstritor associado, reações adversas e, com isso, indicações. Com isso o profissional deve escolher a solução anestésica de acordo com o paciente e procedimento que irá realizar.
