Family planning in the Unified Health System: Use of the intrauterine device

Resumo: Introduction: The Intrauterine Device (IUD) is a long-acting reversible contraceptive method, which causes an inflammatory reaction, developing biochemical and morphological changes, which can trigger minimal tissue damage, which make the environment sterile, altering the cervical mucus and acting as a spermicide, preventing pregnancy, so that ovulation is not affected. Objective: To analyze the insertion and removal of the IUD by the Unified Health System during the years 2020 to 2021. Methodology: Ecological study with an exploratory and quantitative approach based on data obtained from the Department of Informatics of the Unified Health System (DATASUS) on the insertion of the IUD in Brazil between August 2020 and August 2021. It is possible to analyze the growth rate of both the insertion rate and the rate of withdrawal of the same, in addition to the rates by Regions, States, Nursing professionals and professionals of Medicine. Results: In the period from August 2020 to August 2021, 53,118 notifications were registered, 87% of which were insertions and 13% were withdrawn. Highlighting the State of São Paulo located in the Southeast region with 17,279 IUD insertion notifications (37.38%). In addition to obtaining insertion data by nurses, of which 3,272 notifications, there were 528 (16%) in 2020 and 2,744 (84%) in 2021. Conclusion: It is necessary to combine the knowledge of professionals with the knowledge transmitted to users to advance in adherence of this contraception mechanism, because the lack of knowledge ends up being one of the main factors that lead to the withdrawal of the method.

Objetivo: Analizar la inserción y retiro del DIU por parte del Sistema Único de Salud durante los años 2020 al 2021. Metodología: Estudio ecológico con enfoque exploratorio y cuantitativo basado en datos obtenidos del Departamento de Informática del Sistema Único de Salud (DATASUS) sobre la inserción del DIU en Brasil entre agosto de 2020 y agosto de 2021. Es posible analizar la tasa de crecimiento tanto de la tasa de inserción como de la tasa de retiro del mismo, además de las tasas por Regiones, Estados, Profesionales de Enfermería y profesionales de la Medicina. Resultados: En el período de agosto de 2020 a agosto de 2021 se registraron 53.118 notificaciones, de las cuales el 87% fueron inserciones y el 13% fueron retiradas. Destacando el Estado de São Paulo ubicado en la región Sudeste con 17.279 notificaciones de inserción de DIU (37,38%). Además de obtener datos de inserción por parte de las enfermeras, de las cuales 3.272 notificaciones, hubo 528 (16%) en 2020 y 2.744 (84%) en 2021. Conclusión: Es necesario combinar el conocimiento de los profesionales con el conocimiento transmitido a los usuarios para avanzar en la adherencia a este mecanismo anticonceptivo, pues el desconocimiento termina siendo uno de los principales factores que conducen al retiro del método.Introdução: O Dispositivo Intrauterino (DIU) é um método contraceptivo reversível de longa ação, que provoca uma reação inflamatória desenvolvendo alterações bioquímicas e morfológicas, que pode desencadear uma mínima lesão tecidual, que tornam o ambiente estéril, alterando o muco cervical e atuando como espermicida, prevenindo a gestação, de forma que a ovulação não seja afetada. Objetivo: Analisar a inserção e a retirada do DIU pelo Sistema Único de Saúde durante os anos de 2020 a 2021. Metodologia: Estudo ecológico com abordagem exploratória e quantitativa com base nos dados obtidos no Departamento de Informática do Sistema Único de Saúde (DATASUS) sobre a inserção do DIU no Brasil entre agosto de 2020 a agosto de 2021. Sendo possível analisar o índice de crescimento tanto da taxa de inserção, quanto da taxa de retirada do mesmo, além das taxas por Regiões, Estados, profissionais de Enfermagem e profissionais de Medicina. Resultados: No período de agosto de 2020 a agosto de 2021 foram registradas 53.118 notificações, sendo 87% de inserção e 13% de retirada. Tendo como destaque o Estado de São Paulo localizado na Região Sudeste com 17.279 notificações de inserção de DIU (37,38%). Além de obter dados de inserção por enfermeiros das quais 3.272 notificações, foram 528 (16%) em 2020 e 2.744 (84%) em 2021.
