Nutritional status of elderly patients with heart disease admitted to an intensive care unit in Santo Antônio de Jesus, Bahia

Resumo: | The objective of this research was to describe the nutritional status of elderly people with heart disease supported in an intensive care unit in the city of Santo Antônio de Jesus, Bahia. This is a cross-sectional, descriptive study carried out from August to December 2019. Clinical, sociodemographic, lifestyle and laboratory information were collected. For anthropometry, measurements of arm circumference (AC), calf circumference (PC) were measured, and mass, height and body mass index (BMI) were estimated and classified. Data expressed as mean ± mean standard deviation (SPD). Pearson's correlation for the APACHE II score vs. Age, WC, AC, BMI and body mass, with a significance level of p < 0.05. Twenty-two elderly people (13 men and 9 women) were evaluated. Majority with incomplete elementary education (n=20), black race/color (n=12) and with an income of less than 1 minimum wage (n=14). Diagnosis of Systemic Arterial Hypertension (n=16), Acute Myocardial Infarction (n=12) and Congestive Heart Failure (n=12). Low weight according to BMI (n=15), normal weight for arm circumference (n=12) and increased risk of malnutrition for calf circumference (n=12). Serum urea levels above recommended. And inverse correlation (p < 0.02) between the APACHE II score in relation to BMI and body mass. In the study, nutritional risk was observed when the PC and BMI were evaluated. Therefore, nutritional monitoring is essential at admission, aiming to detect risks and intervene with appropriate diet therapy. |

Se trata de un estudio descriptivo transversal realizado de agosto a diciembre de 2019. Se recopiló información clínica, sociodemográfica, de estilo de vida y de laboratorio. Para la antropometría se midieron las medidas de circunferencia del brazo (CA), circunferencia de la pantorrilla (PC), y se estimó y clasificó la masa, la talla y el índice de masa corporal (IMC). Datos expresados como media ± desviación estándar media (SPD). Correlación de Pearson para el puntaje APACHE II vs. Edad, CC, CC, IMC y masa corporal, con un nivel de significación de p < 0,05. Se evaluaron 22 ancianos (13 hombres y 9 mujeres). Mayoría con educación básica incompleta (n=20), raza/color negra (n=12) y con ingresos inferiores a 1 salario mínimo (n=14). Diagnóstico de Hipertensión Arterial Sistémica (n=16), Infarto Agudo de Miocardio (n=12) e Insuficiencia Cardiaca Congestiva (n=12). Bajo peso según IMC (n=15), normopeso para perímetro braquial (n=12) y mayor riesgo de desnutrición para perímetro de pantorrilla (n=12). Niveles de urea sérica por encima de lo recomendado. Y correlación inversa (p < 0,02) entre la puntuación APACHE II en relación al IMC y la masa corporal. En el estudio se observó riesgo nutricional al evaluar la PC y el IMC. Por lo tanto, el seguimiento nutricional es fundamental al ingreso, con el objetivo de detectar riesgos e intervenir con una terapia dietética adecuada.O objetivo dessa pesquisa foi descrever o estado nutricional de idosos cardiopatas admitidos em uma unidade de terapia intensiva no município de Santo Antônio de Jesus, Bahia. Trata-se de um estudo transversal, descritivo, realizado no período de agosto a dezembro de 2019. Foram coletadas informações clínicas, sociodemográficas, hábitos de vida, e laboratoriais. Para a antropometria, foram aferidas as medidas de circunferência do braço (CB), da panturrilha (CP) e foi estimado a massa, estatura e índice de massa corporal (IMC) e classificados. Dados expressos como média ± desvio padrão da média (DPM). A correlação de Pearson, para o escore APACHE II vs. Idade, CP, CB, IMC e massa corporal, com nível de significância de p < 0,05. Foram avaliados 22 idosos (13 homens e 9 mulheres). Maioria com ensino fundamental incompleto (n=20), raça/cor negra (n=12) e com renda menor que 1 salário mínimo (n=14). Diagnóstico de Hipertensão Arterial Sistêmica (n=16), Infarto Agudo do Miocárdio (n=12) e Insuficiência Cardíaca Congestiva (n=12). Baixo peso de acordo com IMC (n=15), eutrofia para a circunferência do braço (n=12) e risco aumentado de desnutrição para a circunferência da panturrilha (n=12). Níveis séricos de uréia acima do recomendado. E correlação inversa (p < 0,02) entre o escore APACHE II em relação ao IMC e massa corporal. No estudo foi observado risco nutricional quando avaliado a CP e o IMC.
