The use of surgical hysteroscopy as an auxiliary method in the diagnosis of adenomyosis. Is it possible?

Resumo: Objective: To address the aspects surrounding the diagnosis of adenomyosis, especially relating the histological findings of biopsies obtained by surgical hysteroscopy with clinical data suggestive of this pathology. Methodology: This is a retrospective observational and cross-sectional study, carried out from the analysis of questionnaires and test results from patients submitted to hysteroscopic biopsy, in a private hospital in the city of Divinópolis-MG. Data analysis was performed, especially between clinical symptoms and confirmation or not of adenomyosis in the anatomopathological samples. Results: It was possible to observe that the most prevalent symptomatology was menorrhagia (76.1%) and menstrual cramps (69.2%), in addition to the fact that the best results of the association of positive hysteroscopic biopsy for adenomyosis with the symptomatology were obtained from the comparison with more than 3 symptoms presented by the patients, with better sensitivity (60.7%), specificity (57.5%) and accuracy (58.8%) and, in the comparison with 3 symptoms, there was the highest negative predictive value (85, 7%). Conclusion: The analysis of biopsy results by surgical hysteroscopy, together with the management of associated symptoms, did not present sufficiently high results for its consolidation as a diagnostic test of adenomyosis in this study. Even so, the scarcity of data in the current literature on this comparison indicates the need to investigate the existence of other possible directions in the still imprecise diagnosis of this pathology, when approached by less invasive methods than hysterectomy.

Metodología: Se trata de un estudio observacional retrospectivo y transversal, realizado a partir del análisis de cuestionarios y resultados de exámenes de pacientes sometidas a biopsia histeroscópica, en un hospital privado de la ciudad de Divinópolis-MG. Se realizó análisis de datos, especialmente entre síntomas clínicos y confirmación o no de adenomiosis en las muestras anatomopatológicas. Resultados: Se pudo observar que la sintomatología más prevalente fue la menorragia (76,1%) y los cólicos menstruales (69,2%), además de que los mejores resultados de la asociación de biopsia histeroscópica positiva para adenomiosis con la sintomatología se obtuvieron de la comparación con más de 3 síntomas presentados por los pacientes, con mejor sensibilidad (60,7%), especificidad (57,5%) y precisión (58,8%) y, en la comparación con 3 síntomas, hubo mayor valor predictivo negativo (85, 7%). Conclusión: El análisis de los resultados de la biopsia por histeroscopia quirúrgica, junto con el manejo de los síntomas asociados, no presentó resultados suficientemente altos para su consolidación como prueba diagnóstica de adenomiosis en este estudio. Aun así, la escasez de datos en la literatura actual sobre esta comparación indica la necesidad de investigar la existencia de otras direcciones posibles en el diagnóstico aún impreciso de esta patología, cuando se aborda por métodos menos invasivos que la histerectomía.Objetivo: Abordar os aspectos que circundam o diagnóstico da adenomiose, sobretudo relacionando os achados histológicos de biópsias obtidas por histeroscopia cirúrgica com dados clínicos sugestivos dessa patologia. Metodologia: Trata-se de um estudo retrospectivo observacional e transversal, realizado a partir da análise de questionários e de resultados de exames de pacientes submetidas à biópsia histeroscópica, em um hospital da rede privada, do município de Divinópolis-MG. Foram feitas análise de dados, sobretudo entre sintomas clínicos e a confirmação ou não de adenomiose nas amostras anatomopatológicas. Resultados: Foi possível observar que a sintomatologia mais prevalente foi menorragia (76,1%) e cólica menstrual (69,2%), além de que os melhores resultados da associação da biópsia histeroscópica positiva para adenomiose com a sintomatologia foram obtidas da comparação com mais de 3 sintomas apresentados pelas pacientes, com melhores sensibilidade (60,7%), especificidade (57,5%) e acurácia (58,8%) e, da comparação com 3 sintomas, houve o maior valor preditivo negativo (85,7%). Conclusão: A análise de resultados de biópsia por histeroscopia cirúrgica, juntamente com a abordagem de sintomas associados não apresentou resultados suficientemente elevados para sua consolidação como um teste diagnóstico de adenomiose neste estudo.
