Dental care for patients with special needs: a literature review

Resumo: The objective of this study was to carry out a narrative bibliographic review on the importance of dental care for people with special needs (PSN), emphasizing attention to the use of medication, restraints, parental care and multidisciplinarity among professionals and family members. Scientific articles in Portuguese, English or Spanish, published from 2000 to 2020, were selected from the databases: PubMED, BBO, LILACS, SCIELO. The research on the platforms took place through the following descriptors: special patients, dental care and oral health. From the data obtained, it was observed that PSN generally manifest more severe oral conditions. These individuals usually have motor limitations resulting from this condition, difficulties in cooperation and aggressive behaviors, which impairs oral hygiene, even though, in most cases, they have a caregiver responsible for it. Thus, dentistry for PSN is a specialty that requires greater selectivity in the techniques that will be used to perform the procedures, in addition to greater attention to the patient's systemic conditions. In addition, there is a need for the dental surgeon to provide the necessary information, demonstrate oral care, indicate auxiliary techniques and, at the same time, encourage caregivers to continue the oral hygiene of these patients.

Los artículos científicos em portugués, inglés o español, publicados entre 2000 y 2020, fueron seleccionados de las bases de datos: PubMED, BBO, LILACS, SCIELO. La investigación em las plataformas se realizó a través de los siguientes descriptores: pacientes especiales, atención dental y salud bucal. A partir de los datos obtenidos, se observó que la PNE generalmente manifiesta condiciones orales más severas. Estas personas generalmente tienen limitaciones motoras como resultado de esta condición, dificultades em la cooperación y comportamientos agresivos, lo que perjudica la higiene bucal, aunque, em la mayoría de los casos, tienen un cuidador responsable de ello. Por lo tanto, la odontología para PNE es una especialidad que requiere una mayor selectividad em las técnicas que se utilizarán para realizar los procedimientos, además de una mayor atención a las condiciones sistémicas del paciente. Además, es necesario que el cirujano dental proporcione la información necesaria, demuestre el cuidado bucal, indique técnicas auxiliares y, al mismo tiempo, aliente a los cuidadores a continuar la higiene bucal de estos pacientes.O objetivo desse estudo foi realizar uma revisão bibliográfica narrativa sobre a importância do atendimento odontológico para portadores de necessidades especiais (PNE), enfatizando a atenção quanto ao uso de medicamentos, contenções, cuidado dos pais e a multidisciplinaridade entre os profissionais e familiares. Foram selecionados artigos científicos nos idiomas português, inglês ou espanhol, publicados de 2000 a 2020, a partir das bases de dados: PubMED, BBO, LILACS, SCIELO. A pesquisa nas plataformas se deu através dos seguintes descritores: pacientes especiais, atendimento odontológico e saúde bucal. A partir dos dados obtidos, observou-se que PNE geralmente manifestam agravos bucais mais severos. Esses indivíduos normalmente apresentam limitações motoras decorrentes dessa condição, dificuldades de cooperação e comportamentos agressivos, o que prejudica a higiene bucal, ainda que, na maioria dos casos, tenha um cuidador responsável pelo mesmo. Dessa forma, a Odontologia para PNE é uma especialidade que requer maior seletividade nas técnicas que serão utilizadas para conseguir realizar os procedimentos, além de uma maior atenção às condições sistêmicas do paciente.
