Impacts of stress on the immune system during the COVID-19 pandemic

Resumo: | The crisis generated by the pandemic impacted several factors in people's daily lives and promoted changes that led to increased stress. This situation draws attention to possible psychiatric complications, not only in patients who have already been infected by the virus, but also in the entire population who had to deal with the measures implemented by the government. Thus, this study is based on evaluating the population's immune compromise associated with stress conditions in the face of the COVID-19 pandemic. This is an integrative literature review, with a qualitative approach based on the collection of national and international data from the following databases: Medical Literature Analysis and Retrieval System Online (MEDLINE/PubMed), Scientific Electronic Library Online (SciELO) and Google Scholar. In the study, the relationship between the nervous, immune and endocrine systems was observed, altering mechanisms in the neuroendocrine immune circuits and causing damage to health due to the stress processes of social isolation. There was increased expression of pro-inflammatory genes and decreased expression of antiviral genes, increasing the risk of complications from COVID-19 due to changes in cytokine transmission. Stress and COVID-19 increase the secretion of IL-6, a cytokine that is associated with death in cases of COVID-19. In health professionals there were changes in immune cells after being exposed to stressors. In view of the results obtained, it was found that stress during the pandemic affected the immune system by altering the response against SARS-CoV-2, increasing viral pathogenesis. |

Esta situación llama la atención sobre posibles complicaciones psiquiátricas, no solo en pacientes que ya han sido infectados por el virus, sino en toda la población que tuvo que lidiar con las medidas implementadas por el gobierno. Así, este estudio se basa en evaluar el compromiso inmunológico de la población asociado a condiciones de estrés ante la pandemia del COVID-19. Esta es una revisión bibliográfica integradora, con un enfoque cualitativo basado en la recopilación de datos nacionales e internacionales de las siguientes bases de datos: Medical Literature Analysis and Retrieval System Online (MEDLINE/PubMed), Scientific Electronic Library Online (SciELO) y Google Scholar. En el estudio se observó la relación entre los sistemas nervioso, inmunológico y endocrino, alterando mecanismos en los circuitos inmunológicos neuroendocrinos y provocando daños a la salud por los procesos de estrés del aislamiento social. Hubo una mayor expresión de genes proinflamatorios y una menor expresión de genes antivirales, lo que aumentó el riesgo de complicaciones por COVID-19 debido a cambios en la transmisión de citoquinas. El estrés y el COVID-19 aumentan la secreción de IL-6, una citocina que se asocia con la muerte en casos de COVID-19. En los profesionales de la salud hubo cambios en las células inmunes después de haber estado expuestos a factores estresantes. A la vista de los resultados obtenidos, se constató que el estrés durante la pandemia afectó al sistema inmunológico alterando la respuesta frente al SARS-CoV-2, aumentando la patogénesis viral.A crise gerada pela pandemia impactou diversos fatores da vida cotidiana das pessoas e promoveu mudanças que propiciaram o aumento do estresse. Esta situação chama atenção para possíveis complicações psiquiátricas, não apenas em pacientes que já foram infectados pelo vírus, mas também em toda a população que precisou lidar com as medidas implementadas pelo governo. Dessa forma, esse estudo baseia-se em avaliar o comprometimento imunológico populacional associado a condições de estresse perante a pandemia da COVID-19. Trata-se de uma revisão integrativa da literatura, com abordagem qualitativa a partir do levantamento de dados nacionais e internacionais das seguintes bases de dados: Medical Literature Analysis and Retrieval System Online (MEDLINE/PubMed), Scientific Eletronic Library Online (SciELO) e Google Scholar. No estudo foi observada a relação do sistema nervoso, imunológico e endócrino, alterando mecanismos nos circuitos imunes neuroendócrino e acarretando danos à saúde pelos processos de estresse de isolamento social. Houve aumento da expressão de genes pró-inflamatórios e diminuição da expressão de genes antivirais aumentando riscos de complicações da COVID-19 devido alterações na transmissão de citocinas. O estresse e a COVID-19 elevam a secreção de IL-6, citocina que está associada à morte nos casos de COVID-19. Em profissionais da saúde houve alterações nas células imunológicas após serem expostos a fatores estressantes.
