Urinary tract infection in the elderly and its antimicrobial susceptibility in the community

Resumo: | Introduction: Urinary tract infection (UTI) is a common disease in the community, especially in the elderly, due to several factors that favor its development and makes treatment difficult. Due to the atypical clinical presentation in this population, the diagnosis becomes more difficult, leading to more severe forms of the disease. A rapid and assertive choice of an antimicrobial therapy becomes essential to restore the health of the elderly. Objective: The purpose of the present study was to review the literature on community-based UTI with a focus on the elderly population, including the analysis of antimicrobial susceptibility. Methodology: This is a literature review, in which articles from the Google Scholar, Scielo, Nature and Science Direct databases and dissertations/thesis and literary works were used in the period from 2015 to 2021. The articles included were available online in Portuguese, English or Spanish and in full text, which reported UTI in the elderly in the community, epidemiological aspect and antimicrobial susceptibility. Results: Among the selected articles, Escherichia coli was the agent with the highest incidence, followed by Klebsiella spp, Proteus mirabilis and Enterococcus spp. and first-generation cephalosporins, and presenting greater sensitivity to beta-lactams, cephalosporins, and carbapenems. Conclusion: Escherichia coli was the most prevalent uropathogen, with different susceptibility indices according to the class of antimicrobials. |

La elección rápida y asertiva de una terapia antimicrobiana se vuelve esencial para restablecer la salud de los ancianos. Objetivo: El propósito del presente estudio fue revisar la literatura sobre la ITU en la comunidad con un enfoque en la población de edad avanzada, incluyendo el análisis de la susceptibilidad antimicrobiana. Metodología: Se trata de una revisión bibliográfica, en la que se utilizaron artículos de las bases de datos Google Scholar, Scielo, Nature y Science Direct y disertaciones/tesis y trabajos literarios en el período comprendido entre 2015 y 2021. Los artículos incluidos estaban disponibles en línea en portugués, inglés o español y en texto completo, que informaron sobre la ITU en los ancianos en la comunidad, el aspecto epidemiológico y la susceptibilidad antimicrobiana. Resultados: Entre los artículos seleccionados, Escherichia coli fue el agente con mayor incidencia, seguido de Klebsiella spp, Proteus mirabilis y Enterococcus spp. y cefalosporinas de primera generación, y presentando mayor sensibilidad a betalactámicos, cefalosporinas y carbapenems. Conclusiones: Escherichia coli fue el uropatógeno más prevalente, con diferentes índices de susceptibilidad según la clase de antimicrobianos.Introdução: A Infecção do trato urinário (ITU) é uma doença frequente especialmente em idosos devido a diversos fatores que propiciam seu desenvolvimento e dificultam o tratamento. Nessa população o quadro clínico é atípico, o que dificulta o diagnóstico, levando a formas mais graves da doença. A instituição da terapia antimicrobiana rápida e assertiva, torna-se essencial para restabelecimento da saúde do idoso. Objetivo: Realizar uma revisão bibliográfica sobre ITU de origem comunitária com foco na população idosa, incluindo a análise da susceptibilidade antimicrobiana. Metodologia: Trata-se de uma revisão bibliográfica, na qual foram utilizados artigos das bases do Google Acadêmico, Scielo, Nature e Science Direct e dissertações/tese e obras literárias no período de 2015 a 2021. Os artigos incluídos estavam disponíveis online em português, inglês ou espanhol, na íntegra, e que abordavam a ITU em idosos na comunidade, aspecto epidemiológico e a susceptibilidade antimicrobiana. Resultados: Dentre os artigos selecionados, a Escherichia coli destacou-se como agente com maior incidência, seguida por Klebsiella spp, Proteus mirabilis e Enterococcus spp.. A análise da susceptibilidade aos antimicrobianos pela Escherichia coli demonstrou o maior índice de resistência a sulfametoxazol-trimetoprim e cefalosporinas de primeira geração, e maior sensibilidade aos beta-lactâmicos, cefalosporinas e carbapenêmicos.
