Pharmacoeconomic analysis of antimicrobials at the intensive care unit in a tertiary hospital

Summary: Antimicrobials are used to inhibit or fight the growth of certain microorganisms and, therefore, they are widely used in the intensive care unit. Pharmacoeconomics is used to improve the use of financial resources, taking the cost-effectiveness of treatment into account. The objective of this work was to analyze the consumption of antimicrobials, identifying the most expensive and most used, in addition to verifying the therapeutic class of greatest consumption. This is a retrospective study with a quantitative approach, carried out from January 2019 to May 2019 in a tertiary hospital. The data were obtained from the forms of use of restricted medications and the database of the hospital. These were recorded in a Microsoft Office Excel 2016® spreadsheet and the expense with antimicrobials was calculated by multiplying the number of units dispensed by the unit price in reais. The analysis of descriptive statistics occurred through the calculation of frequency and percentage. In the period analyzed, 10,286 units of antimicrobials were consumed, corresponding to a total cost of R$ 269,265.94 reais. The most expensive drugs were, respectively, piperacillin + tazobactam, meropenem and linezolid; the most consumed were meropenem, piperacillin + tazobactam and vancomycin, respectively. The classes with the highest consumption were, respectively, carbapenems, penicillin and glycopeptides. From the results, it was verified that the consumption of antimicrobials in the intensive care unit generates a high financial impact on the hospital, in addition to the fact that the most consumed antimicrobials that generated the highest costs were those of broad-spectrum.

La farmacoeconomía se utiliza para mejorar el uso de los recursos financieros, teniendo en cuenta la rentabilidad del tratamiento. El objetivo de este trabajo fue analizar el consumo de antimicrobianos, identificando los más caros y más utilizados, además de verificar la clase terapéutica de mayor consumo. Este es un estudio retrospectivo con un enfoque cuantitativo, llevado a cabo desde enero de 2019 hasta mayo de 2019 en un hospital terciario. Los datos se obtuvieron de las formas de uso de medicamentos restringidos y la base de datos del hospital. Estos se registraron en una hoja de cálculo de Microsoft Office Excel 2016® y el gasto con antimicrobianos se calculó multiplicando el número de unidades distribuidas por el precio unitario en reales. El análisis de estadísticas descriptivas se realizó a través del cálculo de frecuencia y porcentaje. En el período analizado, se consumieron 10.286 unidades de antimicrobianos, lo que corresponde a un costo total de R $ 269.265,94 reales. Los medicamentos más caros fueron, respectivamente, piperacilina + tazobactam, meropenem y linezolid; los más consumidos fueron meropenem, piperacilina + tazobactam y vancomicina, respectivamente. Las clases con mayor consumo fueron, respectivamente, carbapenems, penicilina y glucopéptidos. A partir de los resultados, se verificó que el consumo de antimicrobianos en la unidad de cuidados intensivos genera un alto impacto financiero en el hospital, además del hecho de que los antimicrobianos más consumidos que generaron los mayores costos fueron los de amplio espectro.Os antimicrobianos são utilizados para inibir ou combater o crescimento de determinados micro-organismos e, portanto, são muito utilizados na unidade de terapia intensiva. A farmacoeconomia é usada para melhorar a utilização dos recursos financeiros, levando em consideração o custo-efetividade do tratamento. O objetivo deste trabalho foi analisar o consumo de antimicrobianos, identificando os mais onerosos e mais utilizados, além de verificar a classe terapêutica de maior consumo. Trata-se de um estudo retrospectivo com abordagem quantitativa, realizado no período de janeiro 2019 a maio de 2019 em um hospital terciário. Os dados foram obtidos a partir dos formulários de utilização de medicamentos restritos e do banco de dados do hospital. Estes foram registrados em planilha eletrônica do Microsoft Office Excel 2016® e o gasto com os antimicrobianos foi calculado multiplicando o número de unidades dispensadas pelo preço da unidade em reais. A análise de estatística descritiva ocorreu através do cálculo da frequência e porcentagem. No período analisado houve consumo de 10286 unidades de antimicrobianos, correspondendo a um custo total de R$ 269.265,94 reais. Os medicamentos mais onerosos foram, respectivamente, piperacilina + tazobactam, meropenem e linezolida; já os mais consumidos foram meropenem, piperacilina + tazobactam e vancomicina, respectivamente. As classes com maior consumo foram respectivamente dos carbapenêmicos, penicilina e glicopeptídios.
