Pharmacoeconomic study of antimicrobial control flow adoption by clinical pharmacy of a university hospital

Resumo: Currently antimicrobial resistance is a serious threat to public health. The increase in the incidence of infections by multidrug-resistant organisms has become a focus of concern regarding patient safety throughout the care cycle. The aim of this study was to evaluate the impact of the implementation of a new flow of antimicrobial dispensing and control in a university hospital. An observational and retrospective descriptive study was conducted with a quantitative approach, characterized as a pharmacoeconomic analysis, in one of the hospitalization posts of the University Hospital (HU) of the Federal University of Piauí (UFPI), located in Teresina, Piauí. A reduction in consumption was observed in 9 of the 12 most prescribed antimicrobials after the implementation of the dispensing flow, except for three antimicrobials, Ciprofloxacin 2mg/ml, Metronidazole 500mg/100ml and Vancomycin 500mg. Before the implementation of the flow, the 12 most prescribed antimicrobials implied 93% of the ATB consumed in the hospital, representing only 76% after the appropriate pharmaceutical interventions, demonstrating efficiency of the methodology adopted. The 12 tMDs most prescribed represented a cost of 98% of the hospital's expenses with antimicrobials, which now represents only 51% of the expenses with these. The reduction in costs observed in the period 2021 compared to 2020 may be directly related to the performance of hospital pharmacists in partnership with multidisciplinary teams and CCIH. The relationship between pharmacoeconomics and the rational use of antimicrobials is evident, so that the consumption of antimicrobials directly affects the costs generated in the hospital.

El aumento de la incidencia de infecciones por organismos multirresistentes se ha convertido en un foco de preocupación con respecto a la seguridad del paciente a lo largo del ciclo de atención. El objetivo de este estudio fue evaluar el impacto de la implementación de un nuevo flujo de dispensación y control antimicrobiano en un hospital universitario. Se realizó un estudio descriptivo observacional y retrospectivo con enfoque cuantitativo, caracterizado como análisis farmacoeconómico, en uno de los puestos de hospitalización del Hospital Universitario (HU) de la Universidad Federal de Piauí (UFPI), ubicado en Teresina, Piauí. Se observó una reducción en el consumo en 9 de los 12 antimicrobianos más prescritos después de la implementación del flujo de dispensación, a excepción de tres antimicrobianos, Ciprofloxacina 2mg/ml, Metronidazol 500mg/100ml y Vancomicina 500mg. Antes de la implementación del flujo, los 12 antimicrobianos más prescritos implicaban el 93% del ATB consumido en el hospital, lo que representa solo el 76% después de las intervenciones farmacéuticas apropiadas, lo que demuestra la eficiencia de la metodología adoptada. Los 12 tMD más prescritos representaron un coste del 98% de los gastos del hospital con antimicrobianos, lo que ahora representa solo el 51% de los gastos con estos. La reducción de costes observada en el periodo 2021 respecto a 2020 puede estar directamente relacionada con el desempeño de los farmacéuticos hospitalarios en colaboración con equipos multidisciplinares y CCIH. La relación entre la farmacoeconomía y el uso racional de los antimicrobianos es evidente, por lo que el consumo de antimicrobianos afecta directamente a los costes generados en el hospital.Atualmente a resistência aos antimicrobianos trata-se de uma grave ameaça à saúde pública. Realizou-se um estudo descritivo observacional e retrospectivo com abordagem quantitativa, caracterizado como uma análise farmacoeconômica, em um dos postos de internação do Hospital Universitário (HU) da Universidade Federal do Piauí (UFPI), localizado em Teresina, Piauí. Observou-se redução de consumo em 9 dos 12 antimicrobianos mais prescritos após a implementação do fluxo de dispensação, com exceção apresentada para três antimicrobianos, Ciprofloxacino 2mg/ml, Metronidazol 500mg/100ml e Vancomicina 500mg. A redução dos custos observada no período de 2021 em relação ao de 2020 pode estar diretamente relacionada à atuação dos farmacêuticos do hospital em parceria com as equipes multidisciplinares e CCIH.
