Stewardship of antimicrobials in an intensive care unit during the COVID-19 pandemic in a private hospital

Resumo: | Antimicrobial management is one of the clinical pharmacist's tools. COVID-19 was one of the great challenges for the entire multiprofessional team, since the pandemic directly affected the tertiary health system, generating impacts in the world in relation to antimicrobial therapy. The role of the pharmaceutical pharmacist is necessary to ensure safety in the rationality of medicines and to contribute to the reduction of antimicrobial resistance and rational use. The objective of the study was related to the follow-up of the clinical pharmacist in patients with a positive diagnosis for COVID-19, supported in an intensive care unit, from March to June 2020, in a private hospital in Campo Grande, Mato Grosso do Sul. It was a descriptive, observational approach study. For data collection, a specific research form was used, filled out from the patient's records. Fifty eight patients participated, being 72.41% male, with a mean age of ±61.01 years. There were 10 pharmaceutical interventions related to prescription, being the most frequent treatment time adjustment (17.44% and 30% of treatment), a cost optimization of 81%, a cost optimization of R$25.195,76. The study concludes that the COVID-19 pandemic was a milestone in health care in general, especially regarding the role of the clinician in the monitoring of antimicrobial therapy, with emphasis on the intensive care unit. |

El manejo antimicrobiano es una de las herramientas del farmacéutico clínico. El COVID-19 fue uno de los grandes desafíos para todo el equipo multiprofesional, ya que la pandemia afectó directamente al sistema terciario de salud, generando impactos significativos en el mundo en relación a la terapia antimicrobiana. El papel del farmacéutico clínico es necesario para garantizar la seguridad en la cadena del medicamento y contribuye a la reducción de las resistencias antimicrobianas y al uso racional. El objetivo del estudio fue relatar el seguimiento del farmacéutico clínico en pacientes con diagnóstico positivo para COVID-19, ingresados en unidad de cuidados intensivos, de marzo a junio de 2020, en un hospital privado de Campo Grande, Mato Grosso do Sul. Fue un estudio observacional descriptivo con enfoque cuantitativo. Para la recolección de datos, se utilizó un formulario específico para la investigación, llenado a partir de los registros de las historias clínicas de los pacientes. Participaron 58 pacientes, siendo el 72,41% de sexo masculino, con una edad media de ±61,01 años. Se analizaron un total de 104 intervenciones farmacéuticas relacionadas con la prescripción de antimicrobianos, siendo el ajuste del tiempo de tratamiento el más frecuente (17,30%), y el ahorro generado fue del 81%, optimización de costos evitables de R$ 25.195,76. El estudio concluye que la pandemia de COVID-19 marcó un hito en la atención de la salud en general, especialmente en el papel del farmacéutico clínico en el seguimiento de la terapia antimicrobiana, con énfasis en la unidad de cuidados intensivos.O gerenciamento de antimicrobianos é uma das ferramentas de atuação do farmacêutico clínico. Atuação do farmacêutico clínico se faz necessária garantir a segurança na cadeia medicamentosa e contribui com a diminuição da resistência antimicrobiana e uso racional. O objetivo do estudo foi relatar o acompanhamento do farmacêutico clínico nos pacientes com diagnóstico positivo para COVID-19, admitidos em uma unidade de terapia intensiva, no período de março a junho de 2020, em um hospital privado de Campo Grande, Mato Grosso do Sul. Tratou-se de um estudo descritivo, observacional de abordagem quantitativa. Para a coleta de dados utilizou-se um formulário específico para a pesquisa, preenchidos a partir dos registros do prontuário dos pacientes. Participaram 58 pacientes, sendo 72,41% do sexo masculino, com média de idade de ±61,01 anos. Foram analisadas 104 intervenções farmacêuticas relacionadas à prescrição de antimicrobianos, sendo o ajuste de tempo de tratamento o mais frequente (17,30%), e a economia gerada foi de 81%, uma otimização de custo evitável de R$ 25.195,76.
