Autism Spectrum Disorder in Children: A Systematic Review of the Increasing Incidence and Diagnosis

Resumo: In several countries, increases have been reported in cases diagnosed as Autism Spectrum Disorder (ASD) in children, a process that brings with it numerous questions, ranging from the screening method to the quality of care by health professionals. The central objective of this systematic review was to bring together the most recent studies focused on the care and primary care of children with ASD, and to conduct a brief analysis on the increasing incidence and its significant associations. Aiming to assess this phenomenon, a search was accomplished in the databases of the USA National Library of Medicine (PubMed) and the Virtual Health Library (BVS) considering the articles published in the last 5 years. The terms “autism”, “diagnosis”, “epidemiology” and “pediatrics” and the descriptor “AND” were used, and 140 articles were found. 4 studies were included to complement the search. After being submitted to exclusion criteria, such as passing through the proposed theme and presenting duplicity, 15 works were selected. The results showed an increase in the incidence of autism, especially in more socioeconomically vulnerable communities, in addition, there were concerns related to the screening, diagnosis and professional qualification processes since medical graduation, and the need for improvements that allow early intervention proper. The investment in research in the health area and in the Unified Health System (SUS) directly in the field of primary care in patients with ASD, proved to be one of the most important measures to be taken, due to the complexity of the disorder due to its heterogeneity and the benefits of early intervention.

El objetivo central de esta revisión sistemática fue recopilar los estudios más recientes centrados en la atención y atención primaria de los niños con TEA y realizar un breve análisis de la creciente incidencia y sus asociaciones significativas. Para evaluar este fenómeno, se realizó una búsqueda en las bases de datos de la Biblioteca Nacional de Medicina de EE. UU. (PubMed) y la Biblioteca Virtual de Salud (BVS), considerando los artículos publicados en los últimos 5 años. Se utilizaron los términos “autism”, “diagnosis”, “epidemiology” y “pediatrics” y el descriptor “AND”, se encontraron 140 artículos. 4 estudios fueron incluidos para complementar la investigación. Después de ser sometidos a criterios de exclusión, como pasar por el tema propuesto y presentar duplicidad, 15 obras fueron seleccionadas. Los resultados mostraron un aumento en la incidencia del autismo, especialmente en las comunidades más vulnerables socioeconómicamente, además, hubo inquietudes relacionadas con los procesos de tamizaje, diagnóstico y calificación profesional desde la graduación médica, y la necesidad de mejoras que permitan una adecuada intervención temprana. La inversión en investigación en el área de la salud y en el Sistema Único de Salud (SUS) directamente en el campo de la atención primaria en pacientes con TEA, resultó ser una de las medidas más importantes a ser tomadas, principalmente por la complejidad del trastorno debido a su heterogeneidad y los beneficios de la intervención temprana.Em vários países foram relatados aumentos nos casos diagnosticados como Transtorno do Espectro Autista (TEA) em crianças, processo esse que traz consigo inúmeras indagações, que vão desde o método de triagem até a qualidade do cuidado pelos profissionais de saúde. O objetivo central desta revisão sistemática foi reunir os estudos mais recentes voltados ao cuidado e atenção primária das crianças com TEA e realizar uma breve análise sobre a crescente incidência e suas associações significativas. Visando avaliar este fenômeno, realizou-se uma busca nas bases de dados da USA National Library of Medicine (PubMed) e da Virtual Health Library (BVS) considerando os artigos publicados nos últimos 5 anos. Utilizaram-se os termos “autism”, “diagnosis”, “epidemiology” e “pediatrics” e o descritor “AND”, sendo encontrados 140 artigos. Incluíram-se 4 estudos para complementar a pesquisa. Após serem submetidos à critérios de exclusão, como perpassar a temática proposta e apresentar duplicidade, 15 trabalhos foram selecionados. Os resultados demonstraram um aumento da incidência de autismo, sobretudo, em comunidades mais vulneráveis socioeconomicamente, além disso, evidenciou-se preocupações relacionadas aos processos de triagem, diagnóstico e qualificação profissional desde a graduação médica, e a necessidade de melhorias que possibilitem uma intervenção precoce adequada.
