Analysis of antimicrobial consumption in critical patients in a reference hospital in Piauí

Resumo: Antimicrobials are one of the most widely prescribed classes of drugs worldwide. Due to the large number of occurrences of Health care-associated infections (HAIs), the most diverse classes of these drugs are used and this problem becomes more serious in the Intensive Care Unit (ICU), which is considered an ideal environment for HAIs. The main objective of this study was to analyze the profile of antimicrobial consumption in patients in the Intensive Care Unit of a state public hospital and the consequences of the use of these drugs on kidney function. It is a retrospective quantitative study, including only information contained in the database of patients admitted from January 1 to March 31, 2019, who consumed at least one antimicrobial. The data were recorded in Microsoft Excel spreadsheets and analyzed using relative and absolute frequencies. A higher consumption of carbapenems and a higher prevalence of the microorganism Acinetobacter baumannii were observed, which confirms its rapid dissemination. 46.94% of the patients had impaired kidney functions. In view of this, the importance of a pharmacist in the ICU is demonstrated as he contributes to the review of pharmacotherapy, besides assisting with strategies in choosing the right antimicrobials at doses tolerated by the patient.

Debido a la gran cantidad de casos de infecciones relacionadas con la atención médica (IHA), se utilizan las clases más diversas de estos medicamentos. Este problema se vuelve más grave en la Unidad de Cuidados Intensivos (UCI), que se considera el entorno ideal para las HAI. El objetivo principal de este estudio fue analizar el perfil del consumo de antimicrobianos en pacientes de la Unidad de Cuidados Intensivos de un hospital público estatal y las consecuencias del uso de estos medicamentos en la función renal. Este es un estudio retrospectivo y cuantitativo, que incluye solo información contenida en la base de datos de pacientes hospitalizados del 1 de enero al 31 de marzo de 2019, que consumieron al menos un antimicrobiano. Los datos se registraron en hojas de cálculo en Microsoft Excel y se analizaron utilizando frecuencias relativas y absolutas. Se observó un mayor consumo de carbapenémicos y una mayor prevalencia del microorganismo Acinetobacter baumannii, lo que confirma su rápida propagación. El 46,94% de los pacientes tenían insuficiencia renal. Por lo tanto, la importancia del farmacéutico en la UCI se muestra cada vez más, ya que contribuye a la revisión de la farmacoterapia, además de ayudar en las estrategias para elegir antimicrobianos con dosis toleradas por el paciente.Os antimicrobianos são uma das classes de medicamentos mais prescritas mundialmente. Devido ao grande número de ocorrências de Infecções Relacionadas à Assistência à Saúde (IRAS), tem-se o uso das mais diversas classes desses fármacos. Essa problemática fica mais séria na Unidade de Terapia Intensiva (UTI), que é considerada o ambiente ideal para as IRAS. O objetivo principal deste estudo foi analisar o perfil do consumo de antimicrobianos em pacientes em Unidade de Terapia Intensiva de um hospital público estadual e as consequências do uso desses fármacos na função renal. Trata-se de um estudo retrospectivo e quantitativo, incluindo apenas informações contidas no banco de dados dos pacientes internados de 01 de janeiro a 31 de março de 2019, que consumiram pelo menos um antimicrobiano. Os dados foram registrados em planilhas no Microsoft Excel e analisados por meio de frequências relativas e absolutas. Foram observados um maior consumo dos carbapenêmicos e uma maior prevalência do microrganismo Acinetobacter baumannii, o que confirma sua rápida disseminação. 46,94% dos pacientes tiveram funções renais afetadas.
