Diagnosis of Insulin Resistance in Polycystic Ovary Syndrome

Resumo: Polycystic Ovarian Syndrome (PCOS) is a gynecological endocrine disorder that affects the ovulation process and mainly the female fertility. About 50% of the population diagnosed with PCOS presents peripheral Insulin Resistance (IR). This article aims to gather articles with information about the relation of PCOS with the development of IR, focusing especially on diagnosis methods and the formulation of a flowchart to help the detection of this condition. Were used as inclusion criteria the articles meta-analysis type and systematic revision from 2018 onwards. Based on the findings of this revision, PCOS is directly related to development of insulin resistance especially in obese patients, because of the liberation of pro-inflammatory cytokines of adipose tissue. The flowchart developed from this article offers the possibility to help the interpretation of the results from laboratory tests and increase the diagnostic speed of the insulin resistance in patients with PCOS. For the diagnostic, are dosed the basal glucose and insulin index, OGTT and HOMA-IR. In all of the cases, it’s required the clinic evaluation and past history for the diagnosis.

Alrededor del 50% de la población diagnosticada con SOP tiene Resistencia periférica a la Insulina (RI). Este trabajo tiene como objetivo recopilar artículos con información sobre la relación entre el SOP y el desarrollo de RI, enfocándose principalmente en métodos de diagnóstico y la formulación de un diagrama de flujo para ayudar en la detección de esta condición. Se utilizaron como criterios de inclusión metanálisis y artículos de revisión sistemática del año 2018. Según los hallazgos de esta revisión, el síndrome de ovario poliquístico está directamente relacionado con el desarrollo de resistencia a la insulina, especialmente en pacientes obesos, debido a la liberación de citocinas proinflamatorias del tejido adiposo. El diagrama de flujo desarrollado en este trabajo ofrece la posibilidad de ayudar en la interpretación de los resultados de las pruebas de laboratorio y aumentar la velocidad del diagnóstico de resistencia a la insulina en pacientes con SOP. Para realizar el diagnóstico se miden los niveles basales de glucosa e insulina, TOTG e índice HOMA. En todos los casos, la evaluación clínica y los antecedentes del paciente son necesarios para finalizar el diagnóstico.A síndrome do ovário policístico (SOP) é um distúrbio endócrino ginecológico que afeta o processo de ovulação e principalmente a fertilidade feminina. Cerca de 50% da população com diagnóstico de SOP apresenta Resistência periférica à Insulina (RI). Este trabalho tem como objetivo reunir artigos com informações a respeito da relação da SOP com o desenvolvimento da RI, focado principalmente nos métodos diagnósticos e a formulação de um fluxograma para auxiliar na detecção desta condição. Foram utilizados como critérios de inclusão os artigos do tipo meta-análise e revisão sistemática a partir do ano de 2018. Com base nos achados desta revisão, a SOP está diretamente relacionada ao desenvolvimento de RI principalmente em pacientes obesos, por conta da liberação de citocinas pró-inflamatórias do tecido adiposo. O fluxograma desenvolvido neste trabalho oferece a possibilidade de auxílio na interpretação dos resultados de exames laboratoriais e aumento na velocidade de diagnóstico da resistência à insulina em pacientes com SOP. Para a realização do diagnóstico são dosados os índices de glicose e insulina basal, TOTG e índice de HOMA.
