Mutations of antiretroviral resistance and genetic diversity of HIV-1 in pregnant women attended at a reference maternity hospital in the State of Piauí, Brazil

Resumo: Objective: to identify HIV-1 subtypes, evaluate the presence of antiretroviral resistance (DRM) mutations and estimate the resistance level of the mutant strains detected in pregnant women attended at the reference maternity in Piauí, northeastern Brazil. Methodology: Of the 873 pregnant women, 17 infected with HIV-1 were selected to participate in this study. Genotyping of HIV-1 was performed using nucleotide sequencing followed by phylogenetic analyzes to determine the viral subtype, including identification using the COMET online tool. DRM were defined using the Calibrated Population Resistance Tool program. Additional analyzes were also performed to check drug resistance in the pre-treatment using the Stanford HIVdb Program. Results: Subtype B was detected in all pregnant women infected with HIV-1. The prevalence of DRM was 11.8%. The L90M and K103N mutations were identified, with high (nelfinavir and nevirapine) and intermediate (indinavir and saquinavir) resistance to different antiretrovirals. Conclusion: This study identified important genetic information on HIV-1 in pregnant women in Piauí, which can assist in directing control and prevention measures for this viral infection.

Metodología: De las 873 mujeres embarazadas, se seleccionaron 17 infectadas por el VIH-1 para participar en este estudio. El genotipado del VIH-1 se realizó mediante secuenciación de nucleótidos seguida de análisis filogenéticos para determinar el subtipo viral, incluida la identificación mediante la herramienta en línea COMET. Los DRM se definieron utilizando el programa Calibrated Population Resistance Tool. También se realizaron análisis adicionales para verificar la resistencia a los medicamentos en el pretratamiento utilizando el Programa Stanford HIVdb. Resultados: Se detectó el subtipo B en todas las mujeres embarazadas infectadas por el VIH-1. La prevalencia de DRM fue del 11,8%. Se identificaron las mutaciones L90M y K103N, con resistencia alta (nelfinavir y nevirapina) e intermedia (indinavir y saquinavir) a diferentes antirretrovirales. Conclusión: Este estudio identificó información genética importante sobre el VIH-1 en mujeres embarazadas en Piauí, que puede ayudar a dirigir las medidas de control y prevención de esta infección viral.Objetivo: identificar os subtipos do HIV-1, avaliar a presença de mutações de resistência aos antirretrovirais (DRM) e estimar o nível de resistência das cepas mutantes detectadas em mulheres gestantes atendidas na maternidade de referência no Piauí, nordeste do Brasil. Metodologia: Das 873 mulheres gestantes, 17 infectadas pelo HIV-1 foram selecionadas para participar deste estudo. A genotipagem do HIV-1 foi realizada usando sequenciamento de nucleotídeos seguida de análises filogenéticas para determinar o subtipo viral, incluindo a identificação usando ferramenta on-line COMET. DRM foram definidas usando o programa Calibrated Population Resistance Tool. Análises adicionais também foram realizadas para verificar a resistência aos medicamentos no pré-tratamento usando o Programa Stanford HIVdb. Resultados: O subtipo B foi detectado em todas as gestantes infectadas pelo HIV-1. A prevalência de DRM foi 11.8%.
