Repercussions of the HIV virus in HIV-positive pregnant women and the challenges faced by them: a literature review

Resumo: HIV is a retrovirus with tropism for CD4+ T lymphocytes, cells of the immune system responsible for coordinating the immune response to infection. HIV-positive pregnant women share the fear of social judgment, which makes it difficult to start treatment and increases the risks of vertical transmission. The objective of this work is to investigate the repercussions of the HIV virus in seropositive pregnant women and antiretroviral therapy, and the factors related to therapeutic adherence and vertical transmission, considering lifestyle and biopsychosocial habits. The methodology consists of an integrative review of qualitative literature on the biopsychosocial repercussions of HIV infection in pregnant women. The descriptors “gestation” was crossed; “HIV”; “antiretroviral therapy”; "treatment"; in the following databases: Virtual Health Library (VHL); National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (SCIELO) and EbscoHost. Pregnancy, with the presence of HIV, poses several challenges to women, among which efforts aimed at preventing mother-to-child transmission of the virus stand out. In addition, prior HIV infection or during pregnancy is a risk factor for pregnancy complications and, therefore, must be addressed at all stages of pregnancy. The challenges faced by these pregnant women go beyond pathophysiological barriers. The vertical transmission of HIV has severe consequences for the mother and child, involving risks of malformation and miscarriage; coverage of outpatient screening is critical to the progress of pregnancy.

Las embarazadas seropositivas comparten el miedo al juicio social, lo que dificulta el inicio del tratamiento y aumenta los riesgos de transmisión vertical. Objetivo de este trabajo es investigar las repercusiones del virus VIH en gestantes seropositivas y la terapia antirretroviral, y los factores relacionados con la adherencia terapéutica y la transmisión vertical, considerando estilos de vida y hábitos biopsicosociales. La metodología consiste en una revisión integradora de literatura cualitativa sobre las repercusiones biopsicosociales de la infección por VIH en mujeres embarazadas. Se cruzaron los descriptores “gestación”; "VIH"; "terapia antirretroviral"; "tratamiento"; en las siguientes bases de datos: Biblioteca Virtual en Salud (BVS); Biblioteca Nacional de Medicina (PubMed MEDLINE), Biblioteca Científica Electrónica en Línea (SCIELO) y EbscoHost. El embarazo, con la presencia del VIH, plantea varios desafíos a las mujeres, entre los que se destacan los esfuerzos encaminados a prevenir la transmisión maternoinfantil del virus. Además, la infección previa por el VIH o durante el embarazo es un factor de riesgo para las complicaciones del embarazo y, por lo tanto, debe abordarse en todas las etapas del embarazo. Los desafíos que enfrentan estas gestantes van más allá de las barreras fisiopatológicas. La transmisión vertical del VIH tiene graves consecuencias para la madre y el niño, implicando riesgos de malformación y aborto espontáneo; la cobertura de la detección ambulatoria es fundamental para el progreso del embarazo.O HIV é um retrovírus com tropismo pelos linfócitos T CD4+, células do sistema imune responsáveis por coordenar a resposta imune à infecção. Gestantes soropositivas compartilham o receio do julgamento social o que dificulta o início do tratamento, e amplia os riscos da transmissão vertical. O objetivo do trabalho é investigar as repercussões do vírus HIV em gestantes soropositivas e da terapia antirretroviral, e os fatores relacionados à adesão terapêutica e à transmissão vertical, considerando hábitos de vida e biopsicossociais. A metodologia consiste em uma revisão integrativa de literatura de natureza qualitativa sobre as repercussões biopsicossociais da infecção por HIV em gestantes. Foi realizado o cruzamento dos descritores “gestação”; “HIV”; “terapia antirretroviral”; “tratamento”; nas seguintes bases de dados: Biblioteca Virtual de Saúde (BVS); National Library of Medicine (PubMed MEDLINE), Scientific Eletronic Library Online (SCIELO) e EbscoHost. A gestação, com a presença do HIV, impõe diversos desafios à mulher, os quais se destacam os esforços visando à prevenção da transmissão maternoinfantil do vírus. Ademais, a infecção por HIV prévia ou durante a gestação se faz como um fator de risco para agravos gestacionais e, portanto, deve ser abordada em todos os estágios da gravidez. Os desafios enfrentados por essas gestantes ultrapassam as barreiras fisiopatológicas.
