Nutritional assistance during the COVID-19 pandemic in patients admitted to intensive care units

Resumo: | Nutritional care has undergone unexpected changes in the pandemic of COVID-19. The objective of this article is to describe the nutritional care of patients admitted to intensive care unit in the first fifteen months of this pandemic. This is a retrospective cohort of critically ill patients with COVID-19 admitted to a referral hospital between March/2020 to June/2021. Clinical (complaints, comorbidities, length of stay and evolution), nutritional (nutritional diagnosis and diet prescribed at admission) and patient's origin were evaluated. The Statistical Package for the Social Sciences was used for data analysis. Categorical variables were described by simple, absolute, and relative frequency, and quantitative variables by mean and standard deviation or median and interquartile range. We studied 643 patients with a mean (standard deviation) age of 63 (16) years, 374 (58.2%) were male. Hypertension was the most frequent comorbidity (61.1%), followed by diabetes mellitus (41.4%). Difficulty breathing (79.8%) and fever (48.7%) were the most frequent complaints. The median (interquartile range) hospital stay was 11 (6 - 18) days, and 570 (85.6%) patients died. Only 300 (46.7%) patients had nutritional diagnosis record, among them, 207 (69.0%) were diagnosed with overweight and 35 (11.7%) with malnutrition. There was a lack of nutritional assistance in critically ill patients with COVID-19 in the first 15 months of the pandemic. We suggest a nutritional assessment model that may contribute to better nutritional assistance in catastrophic situations, such as the one experienced in this pandemic. |

El objetivo de este artículo es describir la asistencia nutricional de los pacientes ingresados en la unidad de cuidados intensivos en los primeros quince meses de esta pandemia. Se trata de una cohorte retrospectiva de pacientes críticos con COVID-19 ingresados en un hospital de referencia entre marzo/2020 y junio/2021. Se evaluaron los aspectos clínicos (quejas, comorbilidades, duración de la estancia y evolución), nutricionales (diagnóstico nutricional y dieta prescrita al ingreso) y el origen del paciente. Para el análisis de los datos se utilizó el Statistical Package for the Social Sciences. Las variables categóricas se describieron mediante la frecuencia absoluta y relativa simple y las cuantitativas mediante la media y la desviación estándar o la mediana y el rango intercuartil. Se estudiaron 643 pacientes, con una edad media (desviación estándar) de 63 (16) años, 374 (58,2%) eran hombres. La hipertensión arterial fue la comorbilidad más frecuente (61,1%), seguida de la diabetes mellitus (41,4%). La dificultad respiratoria (79,8%) y la fiebre (48,7%) fueron las quejas más frecuentes. La mediana (rango intercuartil) de la estancia hospitalaria fue de 11 (6 - 18) días y 570 (85,6%) pacientes murieron. Sólo 300 (46,7%) pacientes tenían registro de diagnóstico nutricional, entre ellos, 207 (69,0%) fueron diagnosticados con sobrepeso y 35 (11,7%) con desnutrición. Los resultados demostraron la deficiencia de la atención nutricional en pacientes críticos con COVID-19 en los primeros 15 meses de la pandemia. Proponemos un modelo de evaluación nutricional que puede contribuir a una mejor asistencia nutricional en situaciones catastróficas, como la vivida en esta pandemia.A assistência nutricional passou por mudanças inesperadas na pandemia da COVID-19. Foram avaliadas características clínicas (queixas, comorbidades, tempo de internamento e evolução), nutricionais (diagnóstico nutricional e dieta prescrita na admissão) e a procedência do paciente. Para análise dos dados foi utilizado o pacote estatístico Statistical Package for the Social Sciences. As variáveis categóricas foram descritas por frequência simples absoluta e relativa e as quantitativas por média e desvio padrão ou mediana e intervalo interquartílico. Foram estudados 643 pacientes com média (desvio padrão) de idade de 63 (16) anos, sendo 374 (58,2%) do sexo masculino. Hipertensão arterial foi a comorbidade mais frequente (61,1%), seguida por diabetes mellitus (41,4%). Dificuldade respiratória (79,8%) e febre (48,7%) foram as queixas mais referidas. O tempo de permanência no hospital teve como mediana (intervalo interquartílico) 11 (6 – 18) dias e 570 (85,6%) dos pacientes foram a óbito. Apenas 300 (46,7%) pacientes tinham registro de diagnóstico nutricional, dentre eles, 207 (69,0%) foram diagnosticados com excesso de peso e 35 (11,7%) com desnutrição. Os resultados demonstraram a deficiência da assistência nutricional em pacientes críticos com COVID-19 nos primeiros 15 meses da pandemia.
