Clinical-epidemiological profile of patients with acute aortic syndromes treated at a reference hospital in Pará

Resumo: Objective: To describe the clinical and epidemiological profile of patients diagnosed with acute aortic syndromes in a cardiovascular reference service in the city of Belém – Pará. Methodology: This is a descriptive study of 12 patients admitted to the Hospital de Clínicas Gaspar Vianna, in the period from March to December 2023. Variables related to age, sex, education, origin, symptoms on admission, characteristic of pain on admission, presence of pulses in the upper and lower limbs, type of aortic syndrome, Stanford and DeBakey classification, were considered. whether surgery was necessary, comorbidities, complications, length of stay and outcomes. Results: Chest pain was present in all patients in the sample and most of them were sudden. There was a pulse in the upper and lower limbs. The most common type of aortic syndrome was acute dissection. Stanford's A rating was the most frequent. In the DeBakey classification there was a greater proportion of classes I and III. Among the participants, 07 required surgery, and the most frequent was Bentall. Among the comorbidities mentioned, SAH had the highest proportion. Sepsis was the most frequent complication, followed by AKI, Hematesis and DHE. Conclusion: Half of the patients had death as an outcome. More studies are needed about this pathology, as well as greater prevention of the disease's risk factors to promote continued education for emergency and first aid teams in order to reduce deaths and complications in patients diagnosed with this syndrome.

Metodología: Se trata de un estudio descriptivo de 12 pacientes ingresados ​​en el Hospital de Clínicas Gaspar Vianna, en el periodo de marzo a diciembre de 2023. Se consideraron variables relacionadas con edad, sexo, educación, procedencia, síntomas al ingreso, características del dolor al ingreso, presencia de pulsos en miembros superiores e inferiores, tipo de síndrome aórtico, clasificación de Stanford y DeBakey. Si la cirugía fue necesaria, comorbilidades, complicaciones, duración de la estancia hospitalaria y resultados. Resultados: El dolor torácico estuvo presente en todos los pacientes de la muestra y la mayoría fue repentino. Había pulso en los miembros superiores e inferiores. El tipo más común de síndrome aórtico fue la disección aguda. La calificación A de Stanford fue la más frecuente. En la clasificación de DeBakey hubo mayor proporción de clases I y III. Entre los participantes, 07 requirieron cirugía y el más frecuente fue Bentall. Entre las comorbilidades mencionadas, la HAS tuvo la mayor proporción. La sepsis fue la complicación más frecuente, seguida de IRA, hematesis y DHE. Conclusión: La mitad de los pacientes tuvieron como resultado la muerte. Se necesitan más estudios sobre esta patología, así como una mayor prevención de los factores de riesgo de la enfermedad para promover la educación continua de los equipos de emergencia y primeros auxilios con el fin de reducir muertes y complicaciones en pacientes diagnosticados con este síndrome.Objetivo: Descrever o perfil clínico e epidemiológico de pacientes diagnosticados com síndromes aórticas agudas em um serviço de referência cardiovascular no município de Belém – Pará. Metodologia: Trata-se de um estudo descritivo de 12 pacientes admitidos no Hospital de Clínicas Gaspar Vianna, no período de março a dezembro de 2023. Foram consideradas variáveis relacionadas a idade, sexo, escolaridade, procedência, sintomas na admissão, característica da dor a admissão, presença de pulsos em membros superiores e inferiores, tipo de síndrome aórtica, classificação de Stanford e DeBakey, se foi necessário cirurgia, comorbidades, complicações, tempo de internação e desfechos. Resultados: A dor torácica esteve presente em todos os pacientes da amostra e em sua maioria foi súbita. Houve presença de pulso em MMSS e MMII. O tipo de síndrome aórtica mais frequente foi a Dissecção aguda. A classificação A de Stanford foi a mais frequente. Na classificação DeBakey houve maior proporção da classe I e III. Entre os participantes, 07 necessitaram de cirurgia, e a mais frequente foi a Bentall. Entre as comorbidades referidas, a HAS apresentou a maior proporção. A sepse foi a complicação mais frequente, seguida do LRA, da Hematese e da DHE. Conclusão: Metade dos pacientes tiveram como desfecho o óbito.
