Chronic Chikungunya patient and an wrong diagnosis of rheumatoid arthritis: a case report

Resumo: | The objective of this work is to describe the chronic evolution of Chikungunya infection, its severity factors and its relationship with autoimmune diseases. The case report refers to an adult female patient diagnosed with rheumatoid arthritis (RA), who complained of pain in her wrists, knees, ankles and elbows, which worsened after the end of an acute case of Chikungunya and there was an increase in joint swelling and morning stiffness, without heat or erythema. It was observed that a mistaken diagnosis of RA was made in the patient, due to a similar clinic, which even showed positivity for rheumatoid factor (RF). Through the results of the exams, it was found that the patient did not have rheumatoid arthritis and that her treatment was inadequate for her real diagnosis, therefore, she did not obtain a satisfactory result. Therefore, the differential diagnosis and therapy of chronic Chikungunya should be kept in mind to facilitate the improvement of patients' quality of life. |

El caso clínico se refiere a una paciente adulta del sexo femenino que refirió haber tenido un caso agudo de Chikungunya hace cuatro meses, la paciente presentaba aumento de la tumefacción articular y rigidez matinal, sin calor ni eritema, además se quejaba de dolor en muñecas, rodillas, tobillos y codos. sin embargo, en una consulta con un ortopedista, se le diagnosticó artritis reumatoide (AR). Se observó que en la paciente se hizo un diagnóstico erróneo de AR, debido a una clínica similar, que incluso mostró positividad para factor reumatoide (FR). Mediante los resultados de los exámenes se comprobó que la paciente no padecía artritis reumatoide y que su tratamiento fue inadecuado para su diagnóstico real, por lo que no obtuvo un resultado satisfactorio. Por tanto, se debe tener presente el diagnóstico diferencial y la terapia del Chikungunya crónico para facilitar la mejora de la calidad de vida de los pacientes.O objetivo deste trabalho é descrever a evolução crônica da infecção por Chikungunya, seus fatores de gravidade e sua relação com doenças autoimunes. O relato de caso faz referência a uma paciente adulta, do sexo feminino, que relatou ter passado por quadro agudo de Chikungunya há quatro meses, a paciente apresentou aumento de edema articular e rigidez matinal, sem calor ou eritema, também se queixava de dores em punhos, joelhos, tornozelos e cotovelos, contudo, em uma consulta com ortopedista foi diagnóstica com artrite reumatoide (AR). Observou-se que foi feito um diagnóstico equivocado de AR na paciente, devido a clínica semelhante com chikungunya, que inclusive mostrou positividade para fator reumatoide (FR). Através dos resultados dos exames, verificou-se que a paciente não apresentava artrite reumatoide e que o tratamento era inadequado para seu real diagnóstico, portanto, a mesma não obteve resultado satisfatório.
