The chemical physical analysis in the urine summary as a triage in clinical management

Resumo: The summary urine test consists of physical-chemical analysis and sedimentoscopy. When the physical-chemical phase is normal, it is discussed not carrying out the sedimentoscopy step. This study aimed to evaluate the importance of normal physicochemical results of urine as a screening test for sedimentoscopy. The results of 3661 routine urine summary tests (medium jet) performed at the Clementino Fraga Laboratory, Fortaleza-Ceará, in January 2018 were analyzed. The collected data were recorded, tabulated and analyzed in the SPSS software version 17.0. It was observed that 77.92% of the urine tests showed no physical-chemical alterations. Of the 2658 urines with normal physicochemical analyses, 87.06% had normal sedimentoscopies within a confidence interval of 85.78% - 88.34%. Analyzing the negative predictive value (NPV) of urine with normal physicochemical and correlating it with the alterations found in the sedimentoscopies, we verified that this method has 99.8% of accuracy for all the elements found. The absence of leukocyte esterase and red blood cells in the physical-chemical analysis has an accuracy for sedimentoscopy of 97.5% and 98.2%, respectively. The present study suggests the use of normal physicochemical analysis as an exclusion factor for sedimentoscopy.

Cuando la fase físico-química es normal, se discute no realizar el paso de sedimentoscopia. Este estudio tuvo como objetivo evaluar la importancia de los resultados fisicoquímicos normales de la orina como prueba de detección para la sedimentoscopia. Se analizaron los resultados de 3661 análisis de orina de rutina (chorro medio) realizados en el Laboratorio Clementino Fraga, Fortaleza-Ceará, en enero de 2018. Los datos recolectados fueron registrados, tabulados y analizados en el software SPSS versión 17.0. Se observó que el 77,92% de los exámenes de orina no presentaron alteraciones físico-químicas. De las 2658 orinas con análisis fisicoquímicos normales, el 87,06% tuvo sedimentoscopias normales dentro de un intervalo de confianza de 85,78% - 88,34%. Analizando el valor predictivo negativo (VPN) de la orina con fisicoquímica normal y correlacionándolo con las alteraciones encontradas en las sedimentoscopias, comprobamos que este método tiene un 99,8% de precisión para todos los elementos encontrados. La ausencia de esterasa leucocitaria y glóbulos rojos en el análisis físico-químico tiene una precisión para sedimentoscopia del 97,5% y 98,2%, respectivamente. El presente estudio sugiere el uso de análisis fisicoquímicos normales como factor de exclusión para la sedimentoscopia.O exame sumário de urina é composto pela análise físico-químico e a sedimentoscopia. Este estudo teve como objetivo avaliar a importância dos resultados  físico-químicos normais da urina como triagem ao exame de sedimentoscopia.  Foram analisados os resultados de 3661 exames de sumário de urina de rotina (jato médio) realizados no Laboratório Clementino Fraga, Fortaleza-Ceará, no mês de janeiro de 2018. Os dados coletados foram registrados, tabulados e analisados no software SPSS versão 17.0. Foi observado que 77,92% dos exames de urina não apresentavam alterações físico-químicas. Nas 2658 urinas com análises físico-químicas normais, 87,06% apresentam sedimentoscopias normais dentro de um intervalo de confiança de 85,78% - 88,34%. Analisando o valor preditivo negativo (VPN) das urinas com físico-químicas normais e correlacionando com as alterações encontradas nas sedimentoscopias, verificamos que este método tem 99,8% de acurácia para todos os elementos encontrados.
