Odontogenic infection in deep cervical spaces: diagnosis and treatment

Resumo: An odontogenic infection can originate from a dental element or its supporting structures. Periodontal diseases, endodontics, defective restorative treatments and caries are associated with its most common causes. When identified in its early stage, treatment may include endodontic intervention, replacement of restorative material, periodontal treatment and localized drainage. The objective of this paper is to report a clinical case of a patient who presented a case of odontogenic infection at an advanced stage, discussing the correct diagnosis, its surgical treatment and pharmacological protocol. Male patient, black, 31 years old,with a history of toothache and submandibular abscess with a 22-day evolution time. When performing contrast-enhanced computed tomography of the face and neck, a collection, suggestive of purulent, was observed in submandibular and supraclavicular fascial spaces. The recommended protocol consisted of exploratory cervicotomy of deep fascial spaces together with drainage, removal of material for culture and broad-spectrum antibiotic therapy. The severities of odontogenic infections that affect deep cervical spaces are known, bringing with them the risk of mediastinitis and airway complications. Therefore, safe handling and the surgeon's mastery of early diagnosis and quick and safe intervention delimit complications and increase the chances of cure.

Las enfermedades periodontales, la endodoncia, los tratamientos restauradores defectuosos y la caries se asocian con sus causas más comunes. Cuando se identifica en su etapa inicial, el tratamiento puede incluir intervención endodóntica, reemplazo de material de restauración, tratamiento periodontal y drenaje localizado. El objetivo de este trabajo es reportar un caso clínico de un paciente que presentó un caso de infección odontogénica en estadio avanzado, discutiendo el diagnóstico correcto, su tratamiento quirúrgico y protocolo farmacológico. Paciente masculino, de raza negra, 31 años, con antecedente de dolor de muelas y absceso submandibular con un tiempo de evolución de 22 días. Al realizar una tomografía computarizada con contraste de rostro y cuello, se observó una colección sugestiva de purulenta en los espacios fasciales submandibular y supraclavicular. El protocolo recomendado consistió en cervicotomía exploratoria de espacios fasciales profundos junto con drenaje, extracción de material para cultivo y antibioterapia de amplio espectro. Se conoce la gravedad de las infecciones odontogénicas que afectan los espacios cervicales profundos, trayendo consigo el riesgo de mediastinitis y complicaciones de las vías respiratorias. Por tanto, el manejo seguro y el dominio del cirujano en el diagnóstico precoz y la intervención rápida y segura delimitan las complicaciones y aumentan las posibilidades de curación.Uma infecção odontogênica pode originar-se de um elemento dentário ou de suas estruturas de suporte. Doenças periodontais, endodônticas, tratamentos restauradores defeituosos e cáries estão associadas às suas causas mais comuns. Quando identificada em seu estágio inicial, o tratamento pode incluir intervenção endodôntica, troca de material restaurador, tratamento periodontal e drenagem localizada. O objetivo deste trabalho é relatar um caso clínico de um paciente que apresentou caso de infecção odontogénica em estágio avançado, discutindo o correto diagnóstico, seu tratamento cirúrgico e protocolo farmacológico. Paciente do sexo masculino, 31 anos de idade, sem comorbidades, com quadro de com história de odontalgia e abscesso submandibular com tempo de evolução de 22 dias. Ao realizar tomografia computadorizada de face e de pescoço com contraste, observou-se coleção, sugestiva de coleção purulenta, em espaços fasciais submandibular e supraclavicular do lado direito. O protocolo preconizado consistiu na cervicotomia exploratória de espaços fasciais profundos concomitantemente à drenagem, retirada de material para cultura, remoção do possível foco dentário infeccioso, irrigação copiosa com soro fisiológico 0,9% e terapêutica antibiótica de amplo espectro. Sabe-se das severidades das infecções odontogênicas que acometem espaços cervicais profundos trazendo consigo complicações das vias áreas e possibilidade de dissiminação para cavidade torácica.
