Association between tuberculosis and prison in Nova Alta Paulista: a time series study

Resumo: | Tuberculosis (TB), one of the leading causes of death worldwide, is caused by bacteria of the Mycobacterium tuberculosis complex. TB transmission is commonly facilitated in the prison environment, due to poor ventilation and overcrowding, among other factors. Nova Alta Paulista is made up of 30 municipalities, nine of which have prison units (PU), therefore, knowing the epidemiology of TB in Nova Alta Paulista and its correlation with the presence of PU is important to contribute to the adoption of public measures of prevention health more effective. This is a descriptive research and time series between 2001 and 2020 to study the association between TB and PU cases. The number of TB cases per municipality was extracted from the informatics department of the Brazilian Unified Health System. TB is more prevalent in municipalities that have PU, and the coefficient for determining the mean number of TB cases in the nine municipalities with PU showed a value 62 times greater than the value for the coefficient for determining the mean number of TB cases in the nine municipalities with PU. 21 municipalities without PU, showing that in these municipalities the chance of acquiring and transmitting tuberculosis is greater. In addition, PU employees can be vehicles for acquiring and transmitting TB to the general population. These findings should contribute to the adoption of public health strategies for a better understanding of the dynamics of this disease and the adoption of more effective measures for TB control. |

La transmisión de la tuberculosis suele facilitarse en el entorno penitenciario, debido a la mala ventilación y el hacinamiento, entre otros factores. La Nova Alta Paulista está compuesta por 30 municipios, nueve de los cuales cuentan con unidades penitenciarias (UP), por lo tanto, conocer la epidemiología de la TB en la Nova Alta Paulista y su correlación con la presencia de la UP es importante para contribuir a la adopción de medidas públicas de prevención salud más eficaz. Se trata de una investigación descriptiva y serie temporal entre 2001 y 2020 para estudiar la asociación entre casos de TB y UP. El número de casos de TB por municipio fue extraído del departamento de informática del Sistema Único de Salud de Brasil. La TB es más prevalente en los municipios que tienen UP, y el coeficiente para determinar el promedio de casos de TB en los nueve municipios con UP mostró un valor 62 veces mayor que el valor del coeficiente para determinar el promedio de casos de TB en los 21 municipios sin UP, mostrando que en estos municipios la posibilidad de adquirir y transmitir tuberculosis es mayor. Además, los empleados de la UP pueden ser vehículos para adquirir y transmitir la TB a la población en general. Estos hallazgos deben contribuir a la adopción de estrategias de salud pública para una mejor comprensión de la dinámica de esta enfermedad y la adopción de medidas más eficaces para el control de la TB.A tuberculose (TB), uma das principais causas de morte em todo o mundo, é causada por bactérias do complexo Mycobacterium tuberculosis. A Nova Alta Paulista é composta por 30 municípios, dos quais nove apresentam unidades prisionais (UP), portanto, conhecer a epidemiologia da TB na Nova Alta Paulista e sua correlação com a presença de UP é importante para contribuir com a adoção de medidas públicas de saúde mais eficazes. Trata-se de uma pesquisa descritiva e de séries temporais entre 2001 e 2020 para estudar a associação entre casos de TB e UP. O número de casos de TB por município foi extraído do departamento de informática do Sistema Único de Saúde do Brasil. A TB é mais prevalente em municípios que apresentam UP, e o coeficiente de determinação da média do número de casos de TB dos nove municípios com UP apresentou valor 62 vezes maior que o valor do coeficiente de determinação da média do número de casos de TB dos 21 municípios sem UP, mostrando que nesses municípios a chance de aquisição e transmissão de tuberculose é maior. Além disso, os funcionários das UP podem ser veículos de aquisição e transmissão da TB para a população em geral.
