Spill and extravasation of antineoplastic chemotherapics in a university hospital

Resumo: Spill and leakage stand out as rare and important adverse events in chemotherapy due to their risks. This study aims to determine the incidences and characterize these events, considering their determinants, patient profile and management according to the institutional protocol. This is a retrospective documentary, descriptive and transversal research, with a quantitative approach, carried out in an institution in Belém-Pará. Data collection covered the period from 2012 to 2019, subsidized by reading order and occurrence books, patient records and filling out a specific instrument. The data were structured in Microsoft Excel 2019 for descriptive and statistical analysis using the Chi-square test with p value <0.001 and the kolmogorov-smirnov test. 26 cases of spillage and 41 leakage were analyzed in 55 patients. The incidences of spillage and leakage were 0.08 and 0.13, respectively. Irritating antineoplastic drugs were more frequent in cases of spillage and vesicants had a higher rate in extravasation events, with emphasis on Paclitaxel. The follow-up to the institutional protocol was the majority, however, the measures were not recorded in detail in the medical records. The differential of this research was the joint analysis of the overflow and spill events, showing their concomitance. It is emphasized that the risks of these AEs must be monitored and duly notified, mainly by nursing professionals, aiming at the quality of health care, as well as the safety of professionals, patients and family members.

Este estudio tiene como objetivo determinar las incidencias y caracterizar estos eventos adversos, considerando sus determinantes, el perfil del paciente y el manejo de acuerdo con el protocolo institucional. Se trata de una investigación documental, descriptiva y transversal retrospectiva, con un enfoque cuantitativo, realizada en una institución en Belém-Pará. La recopilación de datos abarcó el período comprendido entre 2012 y 2019, subsidiado por la lectura de libros de orden y ocurrencia, registros de pacientes y el llenado de un instrumento específico. Los datos se estructuraron en Microsoft Excel 2019 para el análisis descriptivo y estadístico utilizando la prueba de Chi-cuadrado con un valor de p <0.001 y la prueba de kolmogorov-smirnov. Se analizaron 26 casos de derrames y 41 fugas en 55 pacientes, considerando que una parte de estos eventos ocurrió concomitantemente. La incidencia de derrames y fugas fue de 0.08 y 0.13, respectivamente. Los fármacos antineoplásicos irritantes fueron más frecuentes en casos de derrame, y los vesicantes tuvieron una tasa más alta de eventos de extravasación, con énfasis en el paclitaxel. El seguimiento del protocolo institucional fue la mayoría, sin embargo, las medidas no se registraron en detalle en los registros médicos. El diferencial de esta investigación fue el análisis conjunto de los eventos de desbordamiento y derrame, mostrando su concomitancia. Se enfatiza que los riesgos de estos EA deben ser monitoreados y debidamente notificados, principalmente por profesionales de enfermería, con el objetivo de la calidad de la atención médica, así como la seguridad de los profesionales, pacientes y familiares.O derramamento e extravasamento destacam-se como eventos adversos raros e importantes na quimioterapia devido aos seus riscos. Foram analisados 26 casos de derramamento e 41 de extravasamento em 55 pacientes. As incidências de derramamento e extravasamento foram 0,08 e 0,13, respectivamente. As drogas antineoplásicas irritantes foram mais frequentes em casos de derramamento e as vesicantes tiveram maior índice nos eventos de extravasamento, com destaque ao Paclitaxel. O seguimento do protocolo institucional foi majoritário, contudo, as medidas não foram registradas em detalhes nos prontuários. O diferencial desta pesquisa se deu pela análise conjunta dos eventos de extravasamento e derramamento, evidenciando sua concomitância.
