The role of the dentist in the diagnosis of mucoepidermoid carcinomas of the parotids glands

Resumo: Mucoepidermoid carcinoma is the most common malignant salivary gland tumor in the oral cavity and affects mainly the parotid gland. Regarding its epidemiology, the incidence of MSC is higher in females and the mean age of patients is 45 years with a peak between the 4th and 6th decades of life. The clinical behavior of MSC is highly variable, ranging from indolent tumor growth to highly aggressive metastatic spread. However, the prognosis will depend considerably on the clinical stage of the lesion, its anatomical location, the histopathological grade and the treatment adopted. Given this, the objective of this study was to discuss the importance of the role of the dentist in the early diagnosis of mucoepidermoid carcinoma in the parotid gland and the result in the prognosis. As for the methodology used, it consists of a narrative literature review, through a search in Google Academic, Scielo, BVS, PUBMED. The diagnostic method is done through palpation, but as there are several differential diagnoses, it is essential that an incisional biopsy is performed for histopathological analysis. It was found that the participation of dentists brought several benefits related to the prognosis of patients with mucoepidermoid carcinoma, due to the possibility of metastasis or the simple fact that it is a malignant tumor and compromises the parotid gland, the dentist's role in diagnosing clinical signs early is essential for a favorable prognosis.

En cuanto a su epidemiología, la incidencia de carcinoma mucoepidermoide es mayor en el sexo femenino y la edad media de los pacientes es de 45 años con un pico entre la 4ª y 6ª década de la vida. El comportamiento clínico de los carcinomas mucoepidermoide es muy variable, desde un crecimiento tumoral indolente hasta una diseminación metastásica muy agresiva. Sin embargo, el pronóstico dependerá considerablemente del estadio clínico de la lesión, su localización anatómica, el grado histopatológico y el tratamiento adoptado. Ante esto, el objetivo de este estudio fue discutir la importancia del papel del odontólogo en el diagnóstico precoz del carcinoma mucoepidermoide de glándula parótida y el resultado en el pronóstico. En cuanto a la metodología utilizada, consiste en una revisión bibliográfica narrativa, a través de una búsqueda en Google Académico, Scielo, BVS, PUBMED.El método diagnóstico se realiza a través de la palpación, pero como existen varios diagnósticos diferenciales, es fundamental que se realice una biopsia incisional para el análisis histopatológico.Se encontró que la participación de los odontólogos trajo varios beneficios relacionados con el pronóstico de los pacientes con carcinoma mucoepidermoide, ya sea por la posibilidad de metástasis o por el simple hecho de que es un tumor maligno y compromete la glándula parótida, el papel del odontólogo en el diagnóstico de signos clínicos temprano es esencial para un pronóstico favorable.O carcinoma mucoepidermóide é o tumor maligno de glândula salivar mais comum na cavidade oral e acomete majoritariamente a glândula parótida. Quanto a sua epidemiologia, a incidência do Carcinoma Mucoepidermóide é maior em indivíduos do sexo feminino e a média de idade dos pacientes é de 45 anos com um pico entre a 4ª e 6ª décadas de vida. O comportamento clínico do Carcinoma Mucoepidermóide é altamente variável, apresentando desde um crescimento tumoral indolente até uma disseminação metastática altamente agressiva. Entretanto, o prognóstico dependerá consideravelmente do estágio clínico da lesão, da sua localização anatômica, do grau histopatológico e do tratamento adotado. Visto isso, o objetivo deste trabalho foi discorrer sobre a importância da atuação do cirurgião-dentista no diagnóstico precoce do carcinoma mucoepidermóide em glândula parótida e a resultância no prognóstico. Quanto à metodologia utilizada consiste em uma revisão de literatura narrativa, por meio da busca nas bases de dados Google acadêmico, Scielo, BVS, PUBMED. O método de diagnóstico é, muitas vezes, feito através da palpação, mas como há diversos diagnósticos diferenciais, é fundamental que seja feita a biópsia incisional para análise histopatológica. Portanto, constatou-se que a participação dos cirurgiões-dentistas trouxe diversos benefícios relacionados ao prognóstico do paciente portador do carcinoma mucoepidermóide, devido a possibilidade de metástase ou pelo simples fato de ser um tumor maligno e de comprometer a glândula parótida.
