Therapeutic itinerary of women with syphilis: from (non)knowledge to choices for health care

Resumo: Therapeutic itineraries consist of the movements triggered by individuals in choosing, evaluating and adhering (or not) to forms of treatment to solve health problems. By understanding that culture determines the type of care desired, it is intended to make nursing practice culturally based on the construction of the therapeutic itinerary of women with syphilis. The objective was to understand the therapeutic itinerary of women diagnosed with syphilis outside the puerperal pregnancy cycle. Based on the Ethnonursing method, carried out in the Municipal STD/AIDS Program in Macaé/RJ. Twenty women diagnosed with syphilis and seven health professionals participated. The data was obtained through the use of two trainers from the Cultural Care Theory, in addition to interviews. The analysis of the testimonies was based on the Ethnonursing method. The results describe the profile of the participants and two analytical categories were coded: Entry into the health care system: building the therapeutic itinerary for syphilis; and the trail of social and cultural interactions for the care of syphilis. It is argued that the therapeutic itinerary of women with syphilis is related to social and cultural issues, leading us to a responsibility as health professionals with the challenge of understanding culture and care as inseparable dimensions. It is concluded that the doubt of women in relation to the context of syphilis is significant. There was a need to discuss the problem of syphilis not only in the puerperal pregnancy cycle, and it is necessary to join efforts to advise and treat this infection in all stages of life.

Al entender que la cultura determina el tipo de cuidado deseado, se pretende hacer la práctica de la enfermería culturalmente a partir de la construcción del itinerario terapéutico de la mujer con sífilis. El objetivo fue conocer el itinerario terapéutico de las mujeres diagnosticadas con sífilis fuera del ciclo del embarazo puerperal. Basado en el método de Etnoenfermería, realizado en el Programa Municipal de ITS/SIDA en Macaé/RJ. Participaron 20 mujeres diagnosticadas de sífilis y siete profesionales de la salud. Los datos se obtuvieron mediante el uso de dos capacitadores de la Teoría del Cuidado Cultural, además de entrevistas. El análisis de los testimonios se basó en el método etnoenfermería. Los resultados describen el perfil de los participantes y se codificaron dos categorías analíticas: Ingreso al sistema de salud: construcción del itinerario terapéutico para la sífilis; y el rastro de interacciones sociales y culturales para el cuidado de la sífilis. Se argumenta que el itinerario terapéutico de las mujeres con sífilis está relacionado con cuestiones sociales y culturales, lo que nos lleva a una responsabilidad como profesionales de la salud con el desafío de entender la cultura y el cuidado como dimensiones inseparables. Se concluye que la duda de las mujeres en relación al contexto de la sífilis es significativa. Era necesario discutir el problema de la sífilis no solo en el ciclo del embarazo puerperal, y es necesario aunar esfuerzos para asesorar y tratar esta infección en todas las etapas de la vida.Itinerários terapêuticos são constituídos pelos movimentos desencadeados por indivíduos na escolha, avaliação e adesão (ou não) a formas de tratamento para resolver os problemas de saúde. Ao compreender que a cultura determina o tipo de cuidado desejado, intui-se tornar a prática da enfermagem culturalmente embasada na construção do itinerário terapêutico de mulheres com sífilis. Objetivou-se compreender o itinerário terapêutico das mulheres com diagnóstico de sífilis fora do ciclo gravídico puerperal. Baseado no método da Etnoenfermagem, realizado no Programa Municipal de DST/AIDS em Macaé/RJ. Participaram vinte mulheres com diagnóstico de sífilis e sete profissionais de saúde. A obtenção dos dados foi através da utilização de dois capacitadores da Teoria do Cuidado Cultural, além de entrevistas. A análise dos depoimentos fundamentou-se no método da Etnoenfermagem. Nos resultados descreve-se o perfil dos participantes e codificou-se duas categorias analíticas: A entrada no sistema de cuidado à saúde: construindo o itinerário terapêutico para a sífilis; e a trilha de interações sociais e culturais para o cuidado com a sífilis. Discute-se que o itinerário terapêutico das mulheres com sífilis está relacionado às questões sociais e culturais, remetendo-nos a uma responsabilidade como profissionais de saúde junto ao desafio de entender a cultura e o cuidado como dimensões inseparáveis. Conclui-se que é significativa a dúvida das mulheres em relação ao contexto da sífilis.
