Analysis of the adequacy of quality indicators in enteral nutrition therapy in an intensive care unit

Resumo: Introduction: Enteral nutritional therapy (ERT) is essential to restore or maintain the patient's nutritional status. However, it may not guarantee the reach of nutritional needs and may generate several complications. Aiming at better patient care, quality tools have been created as quality indicators in enteral nutritional therapy (IQTNE). Objectives: To analyze IQTNE in an intensive care unit (ICU). Methods: This was a retrospective study composed of data collected during eight months from the medical charts of patients under ERT who were admitted to an ICU of a private hospital. The IQTNE analyzed were: (1) frequency of nutritional screening; (2) frequency of days of adequate energy administration; (3) frequency of days of adequate protein administration; (4) frequency of diarrhea; (5) inadvertent output frequency of nutrition probe; (6) frequency of nutrition probe obstruction and (7) adequacy rate of volume infused over prescribed. Results: The sample included data from 122 patients under ERT with mean age of 76.9 ± 16.4 years and 29.9 ± 28.6 days of hospitalization. The IQTNE (1) showed a mean of 97.3 ± 3.1%, IQTNE (2) 88.1 ± 3.8%, IQTNE (3) 89.8 ± 4.8%, IQTNE (4) 9 , IQTNE (5) 3.9 ± 2.2%, with inadequacy in 25% of the months analyzed, IQTNE (6), 0.8% ± 1.5% and the IQTNE (7) obtained 94.3 ± 3.3% of adequacy. The observed complications negatively influenced the adequacy of the instituted ERT. Conclusion: On average, all IQTNs analyzed reached the pre-established goal; however, attention should be given to the symptoms of diarrhea and inadvertent outflow of the nutrition tube.

Sin embargo, puede no garantizar el alcance de las necesidades nutricionales y puede generar varias complicaciones. Con el objetivo de mejorar la atención al paciente, se crearon herramientas de calidad como indicadores de calidad en la terapia de nutrición enteral (ICTNE). Objetivos: Analizar ICTNE en una unidad de cuidados intensivos (UCI). Métodos: Este fue un estudio retrospectivo compuesto por datos recopilados durante ocho meses de los registros de pacientes con TNE que ingresaron en una UCI de un hospital privado. Los ICTNE analizados fueron: (1) frecuencia de cribado nutricional; (2) frecuencia de días de gestión energética adecuada; (3) frecuencia de días de administración adecuada de proteínas; (4) frecuencia de diarrea; (5) frecuencia involuntaria de salida de la sonda de nutrición; (6) frecuencia de obstrucción del tubo de nutrición y (7) tasa de adecuación del volumen infundido en relación con lo prescrito. Resultados: La muestra incluyó datos de 122 pacientes con TNE con una edad media de 76.9 ± 16.4 años y 29.9 ± 28.6 días de hospitalización. El ICTNE (1) mostró un promedio de 97.3 ± 3.1%, el ICTNE (2) 88.1 ± 3.8%, el ICTNE (3) 89.8 ± 4.8%, el ICTNE (4) 9 , 2 ± 6.3%, sin embargo, obtuvo incumplimiento en el 50% de los meses analizados, ICTNE (5) 3.9 ± 2.2%, con insuficiencia en el 25% de los meses analizados, ICTNE (6), 0.8 ± 1.5% y el ICTNE (7) obtuvo 94.3 ± 3.3% de adecuación. Las complicaciones observadas influyeron negativamente en la adecuación de la TNE instituida. Conclusión: en promedio, todos los ICTNE analizados alcanzaron el objetivo preestablecido, sin embargo, se debe prestar atención a los síntomas de diarrea y la retirada involuntaria del tubo de nutrición.Introdução: A terapia nutricional enteral (TNE) é essencial para recuperar ou manter o estado nutricional do paciente. Visando o melhor atendimento ao paciente, foram criadas ferramentas de qualidade como os indicadores de qualidade em terapia nutricional enteral (IQTNE). Objetivos: Analisar IQTNE em uma unidade de terapia intensiva (UTI). Métodos: Estudo retrospectivo composto por dados coletados durante oito meses, dos prontuários de pacientes sob TNE que estiveram internados em uma UTI de um hospital privado. Os IQTNE analisados foram: (1) frequência de realização de triagem nutricional; (2) frequência de dias de administração adequada de energia; (3) frequência de dias de administração adequada de proteína; (4) frequência de diarreia; (5) frequência de saída inadvertida de sonda de nutrição; (6) frequência de obstrução de sonda de nutrição e (7) taxa de adequação do volume infundido em relação ao prescrito. Resultados: A amostra incluiu dados de 122 pacientes sob TNE com média de idade de 76,9±16,4 anos e 29,9±28,6 dias de internamento. O IQTNE (1) evidenciou média de 97,3±3,1%, o IQTNE (2) 88,1±3,8%, IQTNE (3) 89,8±4,8%, o IQTNE (4) 9,2±6,3%, contudo, obteve não conformidade em 50% dos meses analisados, IQTNE (5) 3,9±2,2%, com inadequação em 25% dos meses analisados, IQTNE (6), 0,8±1,5% e o IQTNE (7) obteve 94,3±3,3% de adequação. As intercorrências observadas influenciaram negativamente a adequação da TNE instituída.
