Quality of Life Assessment in patients rehabilitated from head and neck cancer: application of Confirmatory Factor Analysis for construct validation

Resumo: Objective: To apply the Confirmatory Factor Analysis (CFA) technique in a quality of life instrument (WHOQOL-bref), validated for the Portuguese language of Brazil, in patients with maxillofacial prosthesis. Method: exploratory study with retrospective collection of 193 patients with maxillofacial prosthesis seen at Fundação Oncocentro de São Paulo, SP, from 2016 to 2019. Demographic data (gender, age over 18 years, federative unit and date of birth) were included to characterize the sample. lifestyle (alcohol and tobacco consumption) and clinics. Medical records with more than 70% of the variables without information were excluded. The WHOQOL-bref instrument was filled in after indicating the prosthesis. Cronbach's alpha was applied to assess internal consistency and Confirmatory Factor Analysis for construct validity. Results: the instrument showed excellent internal consistency regarding its overall score (alpha = 0.82). For Confirmatory Factor Analysis, the parameters showed the feasibility of the analysis (higher = 0.50). Conclusion: the analysis showed that the WHOQOL-bref instrument can be applied to assess quality of life in patients with maxillofacial prosthesis. The use of this instrument allows the comparison of quality of life between different pathologies, as well as evaluation and monitoring of therapeutic processes.

Método: estudio exploratorio con colección retrospectiva de 193 pacientes con prótesis maxilofacial, de ambos sexos, mayores de 18 años, atendidos en Fundação Oncocentro de São Paulo, SP, en el período de 2016 a 2019. El instrumento WHOQOL-bref se completó después de que se indicó la prótesis. Se aplicó el alfa de Cronbach para evaluar la consistencia interna y el Análisis Factorial Confirmatorio para la validez de constructo. Resultados: el instrumento mostró una excelente consistencia interna con respecto a su puntuación global (alfa = 0,82). Para el Análisis Factorial Confirmatorio, los parámetros mostraron la viabilidad del análisis (mayor = 0,50). Conclusión: el análisis mostró que el instrumento WHOQOL-bref se puede aplicar para evaluar la calidad de vida en pacientes con prótesis maxilofacial. El uso de este instrumento permite la comparación de la calidad de vida entre diferentes patologías, así como la evaluación y seguimiento de procesos terapéuticos.Objetivo: Avaliar a QV de pacientes em uso de prótese bucomaxilofacial, bem como a confiabilidade e a validade dos constructos do WHOQOL-bref. Método: estudo transversal com 193 pacientes, de ambos os sexos, acima de 18 anos, atendidos na Fundação Oncocentro de São Paulo, SP, no periodo de 2016 a 2019. O teste de Mann-Whitney foi aplicado para avalair a diferença entre as variáveis demográficas e clínicas versus os domínios do WHOQOL-bref. Na análise de validação foram aplicados os testes de alfa de Cronbach e a Análise Fatorial Confirmatória. Resultados: Houve diferença entre os sexos para os domínios Psicológico e Ambiental (p<0,050). Para o escore Ambiente, pacientes com 60 anos ou mais apresentaram valores superiores para QV (p<0,010). Para todos os domínios foram observados escores mais elevados na ausência de uso de medicamentos e de comorbidades. Na análise de validação, o instrumento apresentou consistência interna quase perfeita no escore geral (αCronbach = 0,82). Para Análise Fatorial Confirmatória, os parâmetros mostraram a exequibilidade da análise (≥ 0,85) Quanto as cargas fatorais, o domínio Social referente as relações pessoais e atividade sexual mostrou resultados ≥ 0,70. Conclusão: Neste estudo os homens apresentaram melhor QV para os domínios psicológico e ambiente, e das variáveis que avaliaram os aspectos clínicos, a ausência de comorbidades esteve relacionada positivamente a melhor QV nos domínios do WHOQOL-bef.
