Effects of prone position on blood gas indices of patients with Covid-19 admitted to an intensive care unit: a systematic review

Resumo: In 2019, an outbreak of infection caused by SARS-CoV-2 began, having been used measures such as pronation to recover patients. in order to analyze the effects of pronation on gasometric blood indices of patients with Covid-19 admitted to an Intensive Therapy Unit, as a way to contribute to improve practices of health professionals, in their use in cases of pulmonary involvement by Covid-19. This is a systematic review, conducted by two independent reviewers, in order to answer the research question: “What are the effects of the prone position on the PaO2/FiO2 ratio in patients with Covid-19 admitted to an intensive care unit (ICU)? “using "PICO" and the PRISMA flow during the Search, selection and analysis processes.It was observed that the prone position had no significance in the PaO2/FiO2 ratio, as well as no relevant differences in the mortality rate and intubation of patients. It is concluded that although these are studies with high levels of evidence, several biases were observed in their production, making it necessary to expand the development of randomized clinical trials on this subject.

Con eso, el objetivo fue analizar los efectos de la pronación en el índice gasométrico de pacientes con Covid-19 hospitalizados en una unidad de cuidados intensivos, como una forma de contribuir a la mejora de las prácticas de los profesionales de la salud en su uso en casos de enfermedad pulmonar. afectación por Covid-19. Esta es una revisión sistemática, realizada por dos revisores independientes, con el fin de responder a la pregunta de investigación: “¿Cuáles son los efectos de la posición prona sobre la relación PaO2/FiO2 en pacientes con Covid-19 ingresados en una unidad de cuidados intensivos (UCI) ? ?”, utilizando PICO y el diagrama de flujo PRISMA durante los procesos de búsqueda, selección y análisis. Se observó que la posición prono no tuvo significancia en la relación PaO2/FiO2, así como tampoco trajo diferencias relevantes en la tasa de mortalidad e intubación de los pacientes. Se concluye que a pesar de ser estudios con altos niveles de evidencia, se observaron varios sesgos en su producción, siendo necesario ampliar el desarrollo de ensayos clínicos aleatorizados con esta temática.Em 2019 iniciou-se um surto de uma infecção causada pelo SARS-CoV-2 sendo utilizadas medidas como a pronação para recuperação dos pacientes. Com isso objetivou-se analisar os efeitos da pronação nos índices gasométricos de pacientes com Covid-19 internados em unidade de terapia intensiva, como forma de contribuir para a melhoria das práticas dos profissionais de saúde na sua utilização em casos de comprometimento pulmonar por Covid-19. Trata-se de uma revisão sistemática, conduzida por dois revisores independentes, a fim de responder à questão de pesquisa: “Quais os efeitos da posição prona na relação PaO2/FiO2 em pacientes com Covid-19 internados em unidade de terapia intensiva (UTI)?”, utilizando o PICO e o fluxograma PRISMA durante os processos de busca, seleção e análises. Observou-se que a posição prona não teve significância na relação PaO2/FiO2, assim como não trouxe diferenças relevantes na taxa de mortalidade e intubação dos pacientes.
