Epidemiological impact of gestational and congenital syphilis and its relationship with infant mortality in the State of Piauí, Brazil

Resumo: This study aimed to identify and evaluate the epidemiological profile of cases of maternal, congenital syphilis and consequent infant deaths resulting from this infection, in the State of Piauí, reported in the period between 2010 and 2018. A retrospective study was carried out, using secondary data from the Health Information and Analysis Center (DATASUS), through the National System of Notifiable Diseases (SINAN) and the Mortality Information System (SIM) of the Piauí State Department of Health (SESAPI). Several important indicators for the disease were correlated, with an increasing number of cases being observed over the years, with a concomitant increase observed with the incidence rate of gestational and congenital syphilis. The number of deaths was high, prenatal care showed a good percentage over the years, although the evidence shows the low quality of this assistance in the aspects: especially late diagnosis during pregnancy and treatment in most cases performed inappropriately . All of these factors culminate in an increase in acquired syphilis, with the possibility of a relationship with the increase in infant mortality. Therefore, it is clear that even with advances in the prevention of syphilis, this disease still affects many women, causing both sequelae and increased infant mortality.

Se realizó un estudio retrospectivo, utilizando datos secundarios del Centro de Información y Análisis de Salud (DATASUS), a través del Sistema Nacional de Enfermedades de Notificación (SINAN) y el Sistema de Información de Mortalidad (SIM) del Departamento de Salud del Estado de Piauí (SESAPI). Se correlacionaron varios indicadores importantes para la enfermedad, con un número creciente de casos observados a lo largo de los años, con un aumento concomitante observado con la tasa de incidencia de sífilis gestacional y congénita. El número de muertes fue alto, la atención prenatal mostró un buen porcentaje a lo largo de los años, aunque la evidencia muestra la baja calidad de esta asistencia en los aspectos: especialmente el diagnóstico tardío durante el embarazo y el tratamiento en la mayoría de los casos realizado de manera inapropiada . Todos estos factores culminan en un aumento de la sífilis adquirida, con la posibilidad de una relación con el aumento de la mortalidad infantil. Por lo tanto, está claro que incluso con los avances en la prevención de la sífilis, esta enfermedad todavía afecta a muchas mujeres, causando secuelas y una mayor mortalidad infantil.Este trabalho objetivou identificar e avaliar o perfil epidemiológico de casos de sífilis materna, congênita e consequentes óbitos infantis decorrentes desta infecção, no Estado do Piauí, notificados no período entre 2010 e 2018. Realizou-se um estudo retrospectivo, a partir de dados secundários do Núcleo de Informação e Análise em Saúde (DATASUS), através do Sistema Nacional de Agravos de Notificação (SINAN) e no Sistema de Informação sobre Mortalidade (SIM) da Secretaria de Estado da Saúde do Piauí (SESAPI). Foram correlacionados vários indicadores importantes para a doença, sendo observado um número crescente de casos ao longo dos anos, sendo observado o aumento concomitante com a taxa de incidência de sífilis gestacional e congênita. A quantidade de óbito se mostrou elevada, o pré-natal apresentou um bom percentual ao longo dos anos embora as evidências constatam a baixa qualidade desta assistência nos aspectos: diagnóstico especialmente tardio durante a gestação e do tratamento em boa parte dos casos realizado de forma inadequada . Todos esses fatores culminam para aumento de sífilis adquirida, com a possibilidade de relação com o aumento da mortalidade infantil.
