Cone morse x external hexagon, advantages and disadvantages in the clinical aspect: literature review

Resumo: Objective: Dental implants are increasingly present in the daily routine of dentistry, with the Morse taper and external hexagon type being the most frequently used. The present work aims to compare, according to a literary review, the clinical aspects between the morse cone and external hexagon systems of dental implants. Methodology: A selection of scientific articles was made from the Bireme, Pubmed and Scielo databases using the descriptors "cone morse", "external hexagon" and "dental implants". Works published between 2006 and 2018 were included. Of the 622 articles found and delimited by the inclusive criteria, 27 articles were selected as a sample, which presented the theme listed for the research and which were discussed in the following topics: a) peri-implantitis; b) mechanics; c) aesthetics. Results: The morse cone system has advantages in terms of interface sealing, which results in less micro-infiltrations by micro-organisms that cause peri-implantation and has superior mechanics when compared to the external hexagon. Conclusion: Although it presents advantages in relation to micro-infiltration and mechanics, the Morse taper system still costs more than the external hexagon and it is up to the patient to take effective care with oral hygiene and consequent maintenance of the implant, regardless of the system used.

El presente trabajo tiene como objetivo comparar, de acuerdo con una revisión literaria, los aspectos clínicos entre el cono morse y los sistemas hexagonales externos de los implantes dentales. Metodología: Se realizó una selección de artículos científicos de las bases de datos Bireme, Pubmed y Scielo utilizando los descriptores "cono morse", "hexágono externo" e "implantes dentales". Se incluyeron obras publicadas entre 2006 y 2018. De los 622 artículos encontrados y delimitados por los criterios inclusivos, se seleccionaron 27 artículos como muestra, que presentaban el tema enumerado para la investigación y que se discutían en los siguientes temas: a) periimplantitis; b) mecánica; c) estética. Resultados: El sistema de cono Morse tiene ventajas en términos de sellado de interfaz, lo que resulta en menos microinfiltraciones por microorganismos que causan periimplantación y tiene una mecánica superior en comparación con el hexágono externo. Conclusión: aunque presenta ventajas en relación con la microinfiltración y la mecánica, el sistema de cono Morse aún cuesta más que el hexágono externo y depende del paciente tener cuidado efectivo con la higiene oral y el consiguiente mantenimiento del implante, independientemente del sistema utilizado.Objetivo: Os implantes dentários são cada vez mais presentes no dia-a-dia clínico da odontologia, sendo os do tipo cone morse e hexágono externo os mais frequentemente utilizados. O presente trabalho visa comparar, de acordo com uma revisão literária, os aspectos clínicos entre os sistemas cone morse e hexágono externo de implantes dentários. Metodologia: Foi feita uma seleção de artigos científicos a partir das bases de dados Bireme, Pubmed e Scielo utilizando os descritores “cone morse”, “hexágono externo” e “implantes dentários”. Foram incluídos trabalhos publicados entre 2006 e 2018. Dos 622 artigos encontrados e delimitados pelos critérios inclusivos, foram selecionados 27 artigos como amostra, que apresentaram a temática elencada para a pesquisa e que foram discutidos nos seguintes tópicos: a) peri-implantite; b)mecânica; c) estética. Resultados: O sistema cone morse apresenta vantagens no que diz respeito ao selamento de interface, o que traz como consequência menos micro infiltrações por micro-organismos causadores de peri-implante e apresenta mecânica superior quando comparado ao hexágono externo.
