Nutritional care protocol for patients with COVID-19 in Intensive Care Units: an experience report

Resumo: | Recommendations to guide the nutritional care of critically ill patients affected by Coronavirus have been published in recent years. However, there are few reports on the establishment of the new routine of nutritional care and monitoring in the hospital setting. The objective of this article is to present the implementation of a protocol with care flow and nutritional routine of a nutrition service, for patients in the intensive care unit of a reference hospital for COVID-19. This is an experience report that presents the routines of the nutrition care service, which were organized for the care of patients during the pandemic of COVID-19, according to recommendations proposed by national and international entities. In the first stage of care, screening for nutritional risk and checking the level of care were performed to guide the frequency of nutritional monitoring. The calorie and protein needs were calculated according to individual nutritional diagnosis. For nutritional therapy use, the oral or enteral route was prioritized for feeding and procedures for special situations, such as prone position, were standardized. In the first 30 days of the hospital's operation, all nutritional assistance was provided non-presentially, with standardized protocols, which were later revised and adapted to be used in face-to-face care, but still without physical contact of nutritionists with patients. Thus, the production and review of institutional flows, adapting them to the scientific updates published during the pandemic, allowed for the qualification of the nutritional assistance provided in the reference hospital for COVID-19. |

Sin embargo, hay pocos informes sobre el establecimiento de la nueva rutina de atención y seguimiento nutricional en el entorno hospitalario. El objetivo de este artículo es presentar la implementación de un protocolo con el flujo de cuidados y la rutina nutricional de un servicio de nutrición para pacientes en la unidad de cuidados intensivos de un hospital de referencia para COVID-19. Este es un informe de experiencia que presenta las rutinas del servicio de atención nutricional, que se organizaron para la atención de los pacientes durante la pandemia de COVID-19, según las recomendaciones propuestas por entidades nacionales e internacionales. En la primera etapa de la atención, se realizó el cribado del riesgo nutricional y la comprobación del nivel de atención para orientar la frecuencia del control nutricional. Las necesidades de calorías y proteínas se calcularon según el diagnóstico nutricional individual. Para el uso de la terapia nutricional, se priorizó la vía oral o enteral para la alimentación y se estandarizaron los procedimientos para situaciones especiales, como la posición prona. En los primeros 30 días de funcionamiento del hospital, toda la asistencia nutricional se realizó de forma no presencial, con protocolos estandarizados, que posteriormente se revisaron y adaptaron para su uso en la atención presencial, pero aún sin contacto físico de los nutricionistas con los pacientes. Así, la elaboración y revisión de los flujos institucionales, adaptándolos a las actualizaciones científicas publicadas durante la pandemia, permitieron calificar la atención nutricional prestada en el hospital de referencia para el COVID-19.Recomendações para orientar a assistência nutricional de pacientes críticos acometidos pelo Coronavírus foram publicadas nos últimos anos. Contudo, há poucos relatos sobre o estabelecimento da nova rotina do cuidado e monitoramento nutricional no âmbito hospitalar. O objetivo deste artigo é apresentar a implementação de protocolo com fluxo de assistência e rotina nutricional de um serviço de nutrição, para pacientes internados em unidade de terapia intensiva de um hospital de referência para COVID-19. Trata-se de um relato de experiência que apresenta as rotinas do serviço de assistência nutricional, que foram organizadas para o atendimento dos pacientes durante a pandemia da COVID-19, conforme recomendações propostas por entidades nacionais e internacionais. Na primeira etapa do cuidado, a triagem de risco nutricional e a checagem do nível de assistência eram realizados para nortear a periodicidade do monitoramento nutricional. As necessidades calóricas e proteicas eram calculadas conforme diagnóstico nutricional individual. Para uso da terapia nutricional, a via oral ou enteral foram priorizadas para alimentação e condutas para situações especiais, como a posição prona, foram padronizadas. Nos primeiros 30 dias de funcionamento do hospital, toda assistência nutricional foi realizada na modalidade não-presencial, com protocolos padronizados, que posteriormente foram revisados e adequados para serem utilizado no atendimento presencial, porém ainda sem o contato físico dos nutricionistas com os pacientes.
