Patient safety in intensive therapy: occurrence of adverse events in a public hospital

Resumo: | Objective: To characterize adverse events in an ICU in a public hospital. Methods: This is a quantitative, descriptive, retrospective study that analyzed admissions to the ICU in a public hospital between November and December 2019. The identification of adverse events occurred from the review of medical records. Statistical analysis using the MS Excel program, and considered the absolute and relative frequencies for categorical variables; and for continuous variables (mean, standard deviation, minimum and maximum), with the results presented in tables. There were 75 adverse events in 56.9% of 47 admissions. As for the harm to the patient, 38.7% were moderate AEs, 36% mild, 21.3% severe and 4% associated with death. Regarding the origin or type of event, the most frequent categories were: a) clinical process/procedure (56% of AEs), 28.6% of which were events with skin lesions, 21.4% related to procedures not performed, 19% events associated with the probe or drains, among others; b) clinical administration (13.3%), 70% of which were related to missed appointments; and c) infection (9.3%), with 57.1% related to bloodstream infection. Final Considerations: There is a high occurrence of adverse events in the studied ICU, which mainly caused moderate damage and are related to routine procedures in the sector, clinical administration and the control of hospital infections. |

Métodos: Se trata de un estudio cuantitativo, descriptivo, retrospectivo que analizó los ingresos a la UCI de un hospital público entre noviembre y diciembre de 2019. La identificación de eventos adversos se dio a partir de la revisión de historias clínicas. Análisis estadístico utilizando el programa MS Excel, y consideró las frecuencias absolutas y relativas para variables categóricas; y para las variables continuas (media, desviación estándar, mínimo y máximo), con los resultados presentados en tablas. Hubo 75 eventos adversos en el 56,9% de 47 ingresos. En cuanto al daño al paciente, el 38,7% fueron EA moderados, el 36% leve, el 21,3% grave y el 4% asociado a muerte. En cuanto al origen o tipo de evento, las categorías más frecuentes fueron: a) proceso / procedimiento clínico (56% de EA), 28,6% de los cuales fueron eventos con lesiones cutáneas, 21,4% relacionados con procedimientos no realizados, 19% eventos asociados a la sonda o desagües, entre otros; b) administración clínica (13,3%), 70% de las cuales se relacionaron con citas perdidas; yc) infección (9,3%), con un 57,1% relacionado con infección del torrente sanguíneo. Consideraciones Finales: Existe una alta ocurrencia de eventos adversos en la UCI estudiada, los cuales causaron principalmente daño moderado y están relacionados con procedimientos rutinarios en el sector, administración clínica y control de infecciones hospitalarias.Objetivo: Caracterizar os eventos adversos de uma UTI em um hospital público. Métodos: Trata-se de um estudo quantitativo, descritivo, retrospectivo que analisou as admissões em UTI em um hospital público entre novembro e dezembro de 2019. A identificação dos eventos adversos ocorreu a partir da revisão dos prontuários. A análise estatística por meio do programa MS Excel, e considerou as frequências absolutas e relativas para as variáveis categóricas; e para as variáveis contínuas (média, desvio padrão, mínimo e máximo), sendo os resultados apresentados em tabelas. Resultados: Ocorreram 75 eventos adversos em 56,9% das 47 admissões. Quanto ao dano ao paciente 38,7% foram EAs moderados, 36% leves, 21,3% grave e 4% associados a óbito. Em relação a origem ou tipo de evento, as categorias mais frequentes foram: a) processo clínico/procedimento (56% dos EAs), sendo destes 28,6% eventos com lesão a pele, 21,4% relacionados a procedimentos não realizados, 19% eventos associados à sonda ou drenos, dentre outros; b) administração clínica (13,3%), sendo 70% deles relacionados a interconsultas não realizadas; e c) infecção (9,3%), sendo 57,1% relacionados a infecção da corrente sanguínea.
