Health Surveillance Policy: evaluation of the process of management and execution of implementation activities in the territory of the Amazônia Paraense

Resumo: This research evaluated the degree of action of the National Health Surveillance Policy (PNVSA) in a medium-sized municipality in Baixo Tocantins, Pará State, Brazil. Its methodological procedures were: the bibliographic, documentary and empirical research, with data collection occurred in December 2020 and January 2021 with the Municipal Health Secretary, Health Surveillance Coordinator and Coordinators of the different Surveillance. The data were analyzed based on the triangulation of methods, univariate and multivariate statistics and scores were obtained from the arithmetic mean. The results indicate that the degree of action of the Health Surveillance was classified as intermediate, while the Epidemiological Surveillance and Environmental Health Surveillance was unsatisfactory, and the action of the Worker's Health Surveillance (a) was nonexistent. Among the critical points for both, the sub-dimensions stand out: material and technological resources; information management, implementation of actions; monitoring and resolution by goals, and process management is the best evaluated indicator. The weaknesses observed are related to the lack of physical resources for administrative and field work, challenges in relation to the municipal VS database and the periodicity of mandatory actions and health promotion, absence of internal evaluation and lack of direct communication channels with the population, in addition, the professionals were dissatisfied with the work. The challenges found reveal the need for multiple initiatives to enable politics in the dynamic and heterogeneous context that are the territories of the Amazon.

Sus procedimientos metodológicos fueron: la investigación bibliográfica, documental y empírica, con recolección de datos ocurrida en diciembre de 2020 y enero de 2021 con la Secretaría de Salud Municipal, Coordinadora de Vigilancia de la Salud y Coordinadores de las diferentes Vigilancias. Los datos fueron analizados en base a la triangulación de métodos, estadísticas univariadas y multivariadas y se obtuvieron puntuaciones a partir de la media aritmética. Los resultados indican que el grado de actuación de la Vigilancia de la Salud se clasificó como intermedio, mientras que la Vigilancia Epidemiológica y la Vigilancia de la Salud Ambiental fueron insatisfactorias, y la acción de la Vigilancia de la Salud del Trabajador (a) fue inexistente. Entre los puntos críticos para ambos, destacan las subdimensiones: recursos materiales y tecnológicos; gestión de la información, implementación de acciones; el monitoreo y la resolución por objetivos, y la gestión de procesos es el indicador mejor evaluado. Las debilidades observadas están relacionadas con la falta de recursos físicos para el trabajo administrativo y de campo, los desafíos en relación con la base de datos municipal VS y la periodicidad de las acciones obligatorias y de promoción de la salud, la ausencia de evaluación interna y la falta de canales de comunicación directa con la población, además, los profesionales se mostraron insatisfechos con el trabajo. Los desafíos encontrados revelan la necesidad de múltiples iniciativas para posibilitar la política en el contexto dinámico y heterogéneo que son los territorios de la Amazonía.Esta pesquisa avaliou o grau de atuação da Política Nacional de Vigilância em Saúde (PNVSA) em um município de médio porte do Baixo Tocantins, estado do Pará, Brasil. Teve como procedimentos metodológicos: a pesquisa bibliográfica, documental e empírica, com a coleta de dados ocorrida nos meses de dezembro de 2020 e janeiro de 2021 junto ao Secretário Municipal de Saúde, Coordenador da Vigilância em Saúde e Coordenadores das distintas Vigilâncias. Os dados foram analisados com base na triangulação de métodos, na estatística univariada e multivariada e os escores foram obtidos a partir da média aritmética. Os resultados apontam que o grau de atuação da Vigilância Sanitária foi classificado como intermediário, enquanto a Vigilância Epidemiológica e Vigilância em Saúde Ambiental foi insatisfatório, sendo inexistente a atuação da Vigilância em Saúde do Trabalhador (a). Entre os pontos críticos para ambas, destacam-se as subdimensões: recursos materiais e tecnológicos; gestão da informação, execução das ações; monitoramento e resolubilidade por metas, sendo a gestão do processo o indicador mais bem avaliado. As fragilidades constatadas relacionam-se com a insuficiência de recursos físicos para o trabalho administrativo e de campo, desafios com relação ao banco de dados da VS municipal e a periodicidade das ações obrigatórias e de promoção em saúde, ausência de avaliação interna e inexistência de canais de comunicação direto com a população, além disso os profissionais encontravam-se insatisfeitos com o trabalho.
