Surgical treatment of periosteal osteoma in mandible – case report

Resumo: Introduction: Osteomas are proliferations of bone tissue, a benign neoplasm clinically consisting of a well-defined mass that can be sessile or pediculated, asymptomatic and which, depending on its location and size, can cause damage to adjacent structures, besides functional and aesthetic changes. Objective: report a case of periosteal compact osteoma in the mandibular region. Case Report: Patient N.B, female, 33 years old, was seen at the Hospital da face (Recife – PE) complaining of an increase in hardened volume on palpation in the left submandibular region. On physical examination, he presented a hardened, sessile and painless lesion, with an evolution time of approximately 1 year. After clinical examination, computadorized tomography was requested for further evaluation. The tomographic image showed the presence of a hyperdense lesion in the region of the left mandibular body. Due to the appearance of the lesion, surgical excision was chosen through submandibular extra-oral access. After excision, the specimen was sent to a histopathological study that identified the lesion as a compact osteoma. Discussion: Anamnesis and physical examination are of paramount importance for the diagnosis of osteoma, as it is an asymptomatic pathology.  The initial treatment approach is monitoring, if the osteoma causes functional or aesthetic disorders, the treatment chosen is surgical excision. Conclusion: Osteoma is a benign neoplasm which in some cases, surgical intervention is necessary due to aesthetic and functional impairment, as reported in the present case.

Objetivo: Discutir un caso clínico de osteoma periosteal compacto en la región mandibular. Informe del caso: el paciente N.B, mujer, de 33 años, fue visto en el Hospital da Face (Recife – PE) quejándose de un volumen endurecido a la palpación en la región submandibular izquierda. En el examen físico, presentó una lesión endurecida, sésil e indolora, con un tiempo de evolución de aproximadamente 1 año. Después del examen clínico, se solicitó una tomografía facial para una evaluación adicional. La imagen tomográfica mostró la presencia de una lesión hiperdensa en la región del cuerpo mandibular izquierdo. Debido a la apariencia de la lesión, se eligió la escisión quirúrgica a través del acceso extraoral submandibular. Después de la escisión, la muestra se envió a un estudio histopatológico que identificó la lesión como un compact osteoma. Discusión: la anamnesis y el examen físico son extremadamente importantes para el diagnóstico del osteoma, ya que es una patología asintomática. La realización inicial del tratamiento es el seguimiento, y si el osteoma causa trastornos funcionales o estéticos, el tratamiento elegido es la escisión quirúrgica. Conclusión: el osteoma es una neoplasia benigna, que en algunos casos, la intervención quirúrgica es necesaria debido al deterioro estético y funcional, como se informó en el presente caso.Introdução: Os osteomas são proliferações de tecido ósseo, neoplasia benigna clinicamente consiste em massa bem delimitada que pode ser séssil ou pediculada, assintomática e que dependendo da sua localização e tamanho, pode acarretar  danos a estruturas adjacentes, alterações funcionais e estéticos dependendo da sua localização. Objetivo: Discutir um relato de caso de osteoma periosteal compacto em região mandibular. Relato de Caso: Paciente N.B, do sexo feminino, 33 anos, foi atendida no Hospital da face (Recife – PE) queixando-se de aumento de volume endurecido à palpação em região submandibular esquerda. Ao exame físico, apresentava uma lesão endurecida, séssil e indolor, com tempo de evolução de aproximadamente 1 ano. Após exame clínico, foi solicitada a tomografia de face para avaliação complementar. A imagem tomográfica evidenciou a presença de uma lesão hiperdensa em região de corpo mandibular esquerda. Mediante o aspecto da lesão, optou-se pela excisão cirúrgica através de acesso extra-oral submandibular. Após excisão, a peça foi encaminhada para estudo anátomo-patológico que identificou a lesão como um Osteoma compacto. Discussão: A anamnese e exame físico são de suma importância para o diagnóstico do osteoma, por se tratar de uma patologia assintomática. A conduta inicial do tratamento é o acompanhamento, e caso o osteoma acarrete transtornos funcionais ou estéticos, o tratamento eleito é a ressecção cirúrgica.
