Pelvic Mapping: the importance of monitoring nervous system structures in the context of deep intestinal endometriosis

Resumo: Introduction: Deep infiltrative endometriosis (PPE) is defined as the presence of stroma and endometrial glands outside the uterine cavity, requiring a surgical approach in some cases. In this context, pelvic neuromonitoring has been used in surgeries for the treatment of EPI, in addition to being suitable for various surgical procedures. Objectives: The present study aims to describe the importance of monitoring nervous system structures in endometriosis. Methods: This is a literature review based on scientific articles indexed in SCIELO, Pubmed and VHL published between 2015 and 2022. Results: Pelvic neuromonitoring enables intraoperative control of nerve functions and can thus reduce the occurrence of damage and contribute significantly to maintain the quality of life of patients. In the small pelvis, the structures of the autonomic nervous system are complex and sometimes difficult to differentiate, which makes pelvic mapping very important in this area. The system is available for use in open surgery, laparoscopic and robotic surgery. Conclusion: Therefore, neuromonitoring has become a great ally in current pelvic surgeries in which it is desired to save the autonomic neural function, as complications, making the procedure more peaceful and safer.

En este contexto, la neuromonitorización pélvica ha sido utilizada en cirugías para el tratamiento de la EPI, además de ser adecuada para diversos procedimientos quirúrgicos. Objetivos: El presente estudio tiene como objetivo describir la importancia de monitorear las estructuras del sistema nervioso en la endometriosis. Métodos: Esta es una revisión de la literatura basada en artículos científicos indexados en SCIELO, Pubmed y BVS publicados entre 2015 y 2022. Resultados: La neuromonitorización pélvica permite el control intraoperatorio de las funciones nerviosas y puede así reducir la ocurrencia de daño y contribuir significativamente a mantener la calidad de vida de los pacientes. En la pelvis pequeña, las estructuras del sistema nervioso autónomo son complejas y en ocasiones difíciles de diferenciar, lo que hace que el mapeo pélvico sea muy importante en esta zona. El sistema está disponible para su uso en cirugía abierta, cirugía laparoscópica y robótica. Conclusión: Por tanto, la neuromonitorización se ha convertido en una gran aliada en las cirugías pélvicas actuales en las que se desea salvar la función neuronal autonómica, como complicaciones, haciendo más tranquilo y seguro el procedimiento.Introdução: A endometriose profunda infiltrativa (EPI) é definida como a presença de estroma e glândulas endometriais fora da cavidade uterina, sendo necessária em alguns casos a abordagem cirúrgica. Neste contexto, a neuromonitorização pélvica tem sido usada nas cirurgias para o tratamento da EPI, além de ser adequada para vários procedimentos cirúrgicos. Objetivos: O presente estudo tem por objetivo descrever a importância da monitorização de estruturas do sistema nervoso no âmbito da endometriose. Métodos: Trata-se de revisão bibliográfica embasada em artigos científicos indexados no SCIELO, Pubmed e BVS publicados entre 2015 e 2022. Resultados: O neuromonitoramento pélvico possibilita o controle intraoperatório das funções nervosas e pode, assim, reduzir a ocorrência de danos e contribuir significativamente para a manutenção da qualidade de vida dos pacientes. Na pelve pequena, as estruturas do sistema nervoso autônomo são complexas e por vezes difíceis de diferenciar, o que torna o mapeamento pélvico muito importante nesta área. O sistema está disponível para uso em cirurgias abertas, cirurgias laparoscópicas e robóticas.
