Physiopathological aspects involved in the symptomatology of COVID-19 and its consequences: a literature review

Resumo: | Introduction: SARS-CoV-2 is a betacoronavirus belonging to the Nidovirales order of the Coronaviridae family. It is a virus that became evolutionarily able to interact with transmembrane enzymes (ACE 2) occuring in human body's cells and, therefore, replicating inside them. SARS-CoV-2 might produce a cytokyne storm on human's organism, causing several systemic damages to the patients. Objective: Describe the main aspects about viral characterization, physiopathology, systemic consequences, as well as the infection's diagnosis. Methodology: It is about a literature review concerning academical articles available on the following database: PubMed, SciElo, UPTODATE and LILACS, using the following descriptors: COVID-19, SARS-CoV-2 and COVID-19 pandemic. Discussion: SARS-CoV-2's cell's entrance may generate multiple inflammatory processes on human's organism. Since there is ACE 2 receptors on many organs cells, the virus is able to produce systemic injuries. Lung lesions can be identified on the main imaging tests there is, thus helping to comprehend the prognosis and treatment of the patients. Due to the virus agression against the organism, many patients complain about some symptoms that remain even after infection's resolution. Final notes: Throughout this study, it was possible to understand historical aspects over coronavirus, its variants, the SARS-CoV-2-induced cytokyne storm, as well as systemic repercussions during and after the infection and the available diagnostic methods too. |

Es un virus que, evolutivamente, se volvió capaz de interactuar con las enzimas transmembrana (ACE2) presentes en las células del cuerpo humano y, en consecuencia, invadirlas y multiplicarse en su interior. El SARS-CoV-2 puede generar una tormenta de citocinas en el organismo, causando diversos daños sistémicos a los infectados. Objetivo: describir los principales aspectos sobre la caracterización del virus, la fisiopatología, las consecuencias sistémicas y el diagnóstico de la infección. Metodología: Se trata de una revisión bibliográfica con levantamiento de textos académicos en las siguientes bases de datos: PubMed, SciElo, UPTODATE y LILACS, con los siguientes descriptores: ''COVID-19'', ''SARS-CoV-2'' y ''pandemia por COVID-19''. Discusión: La invasión del SARS-CoV-2 al interior de las células genera varios procesos inflamatorios en el cuerpo humano. Como hay receptores ACE2 en células de diferentes órganos, el virus es capaz de causar lesiones sistémicas. Las lesiones pulmonares pueden identificarse en las principales pruebas de imagen, ayudando así a entender el pronóstico y tratamiento de los pacientes. Debido a las agresiones al organismo humano provocadas por el virus, muchos pacientes se quejan de algunos síntomas que persisten incluso después de curar la infección. Consideraciones finales: A través de este estudio se logró comprender aspectos históricos sobre el coronavirus, sus variantes, estructura, la fisiopatología de la infección, la tormenta de citoquinas que genera, así como las consecuencias sistémicas durante y después del proceso infeccioso y los posibles métodos de diagnóstico.Introduçao: O SARS-CoV-2 é um betacoranavírus pertencente à ordem Nidovirales da família Coronaviridae. Ele é um vírus que, evolutivamente, se tornou capaz de interagir com as enzimas transmembranas (ECA2) presentes em células do corpo humano e, consequentemente, invadir e se multiplicar dentro delas. O SARS-CoV-2 pode gerar uma tempestade de citocinas no organismo, causando vários danos sistêmicos aos infectados. Objetivo: descrever os principais aspectos sobre a caracterização do vírus, a fisiopatologia, as consequências sistêmicas e o diagnóstico da infecção. Metodologia: Trata-se de uma revisão bibliográfica com levantamento de textos acadêmicos nas seguintes bases de dados: PubMed, SciElo, UPTODATE e LILACS, com os seguintes descritores: ‘‘COVID-19’’, ‘‘SARS-CoV-2’’ e ‘‘pandemia por COVID-19’’. Discussão: A invasão do SARS-CoV-2 dentro das células gera vários processos inflamatórios no organismo humano. Como existem receptores ECA2 em células de diversos órgãos, o vírus é capaz de causar lesões sistêmicas. As lesões pulmonares podem ser identificadas nos principais exames de imagem, ajudando, assim, a entender o prognóstico e o tratamento dos pacientes. Devido às agressões ao organismo humano causadas pelo vírus, muitos pacientes se queixam de alguns sintomas que persistem mesmo após a cura da infecção.
