Use of heart rate monitors to measure heart rate variability at rest: a literature review

Resumo: The Heart Rate Variability (HRV) is a non-invasive method to estimate the cardiac autonomic control that can be measured through the electrocardiogram (ECG), the gold standard method. Portable devices that are more practical and cheaper than ECG, such as heart rate monitors, are a more convenient alternative for measuring HRV in clinical practice, but it is necessary that their results be validated in different populations. Objective: To analyze the studies carried out on the validity of heart rate monitors in comparison with the ECG for measuring HRV at rest in different populations. Methodology: This is a literature review where studies published in the Pubmed and Scielo databases were used, no lower limit of the publication date until the year 2021. The following descriptors were used in English and Portuguese: "heart rate variability" or "cardiac autonomic control" or "Electrocardiography" or "Heart Rate", being associated with the terms "heart rate monitor" and "reproducibility" or "validity" or "agreement". Results and discussion: Fifteen studies were found that evaluated the validity of different models of heart rate monitors for measuring HRV at rest, compared to ECG, in different populations (healthy adults, athletes, elderly, children, obese adolescents, individuals with injuries spinal cord and elderly), as well as its reliability in the test-retest of some devices. Conclusion: Studies have shown that different heart rate monitor models can be used as an alternative to ECG for measuring HRV, even in different populations.

Los dispositivos portátiles más prácticos y económicos que el ECG, son una alternativa más conveniente para medir la VFC en la práctica clínica, pero es necesario que sus resultados estén validados en diferentes poblaciones. Objetivo: Analizar los estudios realizados sobre la validez del uso de pulsómetros en comparación con el ECG para medir la VFC en reposo en diferentes poblaciones. Metodología: Se trata de una revisión de la literatura donde se utilizaron estudios publicados en las bases de datos Pubmed y Scielo, sin límite inferior de la fecha de publicación hasta el año 2021. Los siguientes descriptores se utilizaron en inglés y portugués: "variabilidad de la frecuencia cardíaca" o "control autónomo cardíaco" o "electrocardiografía" o "frecuencia cardíaca", estando asociado con los términos "monitor de frecuencia cardíaca" y "reproducibilidad" o "validez" o "acuerdo". Resultados y discusión: Se encontraron quince estudios que evaluaron la validez de diferentes modelos de monitores de frecuencia cardíaca para medir la VFC en reposo, en comparación con el ECG, en diferentes poblaciones (adultos sanos, deportistas, ancianos, niños, adolescentes obesos, individuos con lesiones medulares y ancianos), así como su fiabilidad en el test-retest de algunos dispositivos. Conclusión: Los estudios han demostrado que se pueden utilizar diferentes modelos de monitor de frecuencia cardíaca como una alternativa al ECG para medir la VFC, incluso en diferentes poblaciones.A Variabilidade da Frequência Cardíaca (VFC) é um método não-invasivo utilizado para estimar o controle autonômico cardíaco que pode ser mensurada através do eletrocardiograma (ECG), método padrão-ouro. Dispositivos portáteis que tragam maior praticidade e com menor custo que o ECG, como os cardiofrequencímetros, são uma alternativa mais conveniente para a mensuração da VFC na prática clínica, porém é necessário que seus resultados sejam validados em diferentes populações. Objetivo: Analisar os estudos realizados sobre a validade dos cardiofrequencímetros em comparação com o ECG para mensuração da VFC no repouso em diferentes populações. Metodologia: Trata-se de uma revisão de literatura onde foram utilizados estudos publicados na base de dados do Pubmed e Scielo, sem limite inferior da data de publicação até o ano de 2021. Foram utilizados os seguintes descritores nos idiomas inglês e português: "heart rate variability" ou "cardiac autonomic control" ou "Electrocardiography" ou "Heart Rate", sendo associados aos termos “heart rate monitor” e "reproducibility" ou “validity” ou “agreement”. Resultados e discussão: Foram encontrados 15 estudos que avaliaram a validade de diferentes modelos de cardiofrequencímetros para a mensuração da VFC no repouso, em comparação com o ECG, em diferentes populações (adultos saudáveis, atletas, idosos, crianças, adolescentes obesos, indivíduos com lesão medular e idosos), assim como sua confiabilidade no teste-reteste de alguns dispositivos.
