Therapeutic possibilities for the treatment of Canine Visceral Leishmaniasis in Brazil: Literature review

Resumo: Canine Visceral Leishmaniasis (CVL) is considered an endemic disease in several regions of Brazil, being the focus of several public policies because of its zoonotic potential. The only medication regulated by the Ministry of Agriculture, Livestock and Supply (MAPA) in the country for the treatment of the disease in animals is miltefosine. Thus, the search for innovative drugs is necessary, some of which are already in phases of laboratory trials and others have been indicated for use in clinical routine in other countries. Therefore, the objective of the present study was to highlight the therapeutic possibilities against CVL, trying to emphasize, mainly, the new drugs frequently used in clinical routine, as well as those that have the potential to become viable options for the approach of CVL in Brazil.  For the review, we implemented a manual systematic search in the main scientific databases, using indexers related to the theme in the period from 1994 to 2022. The results of the systematic search revealed that the therapeutic options potentially active against Leishmania infantum include marbofloxacin, beta-glucans, immunomodulators, and essential oils from medicinal plants and their derivative compounds, which are considered promising candidates for the treatment and control of CVL.  Therefore, from the data gathered, it was possible to verify that there are many studies about the therapeutics of the disease and that understanding them can be decisive in providing good care to the sick animal, as well as in the implementation of an adequate drug planning.

El único medicamento regulado por el Ministerio de Agricultura, Ganadería y Abastecimiento (MAPA) en el país para el tratamiento de la enfermedad en los animales es la miltefosina. Por ello, es necesaria la búsqueda de fármacos innovadores, algunos de los cuales ya se encuentran en fase de ensayos de laboratorio y otros han sido indicados para su uso en la rutina clínica en otros países. Por lo tanto, el objetivo del presente estudio fue destacar las posibilidades terapéuticas contra la CVL, tratando de enfatizar, principalmente, los nuevos fármacos frecuentemente utilizados en la rutina clínica, así como aquellos que tienen el potencial de convertirse en opciones viables para el abordaje de la CVL en Brasil.  Para la revisión, se implementó una búsqueda sistemática manual en las principales bases de datos científicas, utilizando indizadores relacionados con el tema en el período comprendido entre 1994 y 2022. Los resultados de la búsqueda sistemática revelaron que las opciones terapéuticas potencialmente activas contra Leishmania infantum incluyen la marbofloxacina, los betaglucanos, los inmunomoduladores, así como los aceites esenciales de plantas medicinales y sus compuestos derivados, que se consideran candidatos prometedores para el tratamiento y el control de la CVL.  Por lo tanto, a partir de los datos recogidos, se pudo comprobar que existen muchos estudios sobre la terapéutica de la enfermedad y que su comprensión puede ser decisiva para proporcionar una buena atención al animal enfermo, así como para la aplicación de un plan farmacológico adecuado.A Leishmaniose Visceral Canina (LVC) é considerada uma enfermidade endêmica em diversas regiões do Brasil, sendo foco de diversas políticas públicas em virtude de seu potencial zoonótico. A única medicação regulamentada pelo Ministério de Agricultura, Pecuária e Abastecimento (MAPA) no país, para o tratamento da enfermidade em animais, é a miltefosina. Desta forma, a busca por fármacos inovadores é necessária, os quais alguns já se encontram em fases de ensaios laboratoriais e outros têm sido indicados para uso na rotina clínica em outros países. Assim sendo, o objetivo do presente trabalho foi evidenciar as possibilidades terapêuticas contra a LVC, buscando ressaltar, principalmente, os novos fármacos frequentemente utilizados na rotina clínica, bem como aqueles que possuem potencial para se tornarem opções viáveis para a abordagem da LVC no Brasil.  Para a revisão, implementou-se uma busca sistemática manual nas principais bases de dados científicos, a partir da utilização de indexadores relacionados à temática no período de 1994 a 2022.
