Resumo: The process in which colorectal cancer (CRC) is metastasized to the ovary remains unclear. However, it is considered that, as there is no lymphatic flow between the colon and the ovaries, both hematogenous and disseminated peritoneal metastases present possible metastatic pathways. This study has as objective to describe the difficulties and how the diagnosis of colorectal cancer with ovarian metastasis is done. This work was carried out through the Virtual Health Library (VHL) at Pubmed and Medline databases, using the crossings of the descriptors in English “Colorectal cancer”, “Ovarian metastasis”, and “Diagnosis”. To evaluate the research problem and its stratification, it was used the PVO strategy, which can be seen in Table 1. Such strategy permitted the formulation of the following guiding question: “How is the diagnosis of colorectal cancer with ovarian metastasis done?”. Differential diagnosis between primary tumors and ovarian metastases is often difficult. Symptoms associated with the development of ovarian cancer can hide an intestinal disease. Serum values of carbohydrate antigen tumor markers (CA-125) and carcinoembryonic antigen (CEA) have an important role in differentiating patients with primary CRC with ovarian metastasis. Although difficult, the differential diagnosis must be done early to minimize complications. Diagnostic alternatives include histopathological evaluation with the identification of metastatic histological subtypes similar to primary colorectal cancer subtypes, being mucinous adenocarcinoma the most common.

Sin embargo, se considera que, al no existir flujo linfático entre el colon y los ovarios, tanto las metástasis peritoneales hematógenas como diseminadas tienen posibles vías metastásicas. El presente estudio tiene como objetivo describir las dificultades y cómo se realiza el diagnóstico de cáncer colorrectal con metástasis de ovario. Realizado a través de la Biblioteca Virtual en Salud (BVS) en las bases de datos Pubmed y Medline, utilizando los cruces de los descriptores en inglés “Colorrectal cancer”, “Ovarian metastasis”, “Diagnosis”. Para evaluar el problema de investigación y su estratificación se utilizó la estrategia OPV, formulándose la siguiente estrategia, que se puede ver en el Cuadro 1. La estrategia mencionada permitió formular la siguiente pregunta orientadora: "¿Cómo es el diagnóstico de cáncer colorrectal con metástasis de ovario? ". El diagnóstico diferencial entre tumores primarios y metástasis ováricas suele ser difícil. Los síntomas asociados con el desarrollo de cáncer de ovario pueden enmascarar la enfermedad intestinal. Los valores séricos de los marcadores tumorales del antígeno carbohidrato (CA-125) y del antígeno carcinoembrionario (CEA) juegan un papel importante en la diferenciación entre pacientes con CCR primario con metástasis ovárica. Aunque es difícil, el diagnóstico diferencial debe realizarse temprano para minimizar las complicaciones. Las alternativas diagnósticas incluyen la evaluación histopatológica con identificación de subtipos histológicos metastásicos similares a los subtipos de cáncer colorrectal primario, siendo los más comunes los adenocarcinomas mucinosos.O processo pelo qual o câncer colorretal (CCR) metastatiza para o ovário permanece obscuro. Porém, considera-se que, como não há fluxo linfático entre o cólon e os ovários, tanto as metástases hematogênicas quanto as peritoneais disseminadas apresentam possíveis vias metastáticas. O presente estudo tem como objetivo descrever as dificuldades e como é feito a realização do diagnóstico do câncer colorretal com metástase de ovário. Realizado através da Biblioteca Virtual de Saúde (BVS) nas bases de dados do Pubmed e Medline, usando os cruzamentos dos descritores em inglês “Colorectal cancer”,“Ovarian metastasis”, “Diagnosis”. Para a avaliação do problema de pesquisa e sua estratificação foi utilizada a estratégia PVO sendo formulada a seguinte estratégia que pode ser observada no Quadro 1. A estratégia supracitada permitiu formular a seguinte questão norteadora: “Como é realizado o diagnóstico do câncer colorretal com metástase de ovário?”. O diagnóstico diferencial entre tumores primários e metástases ovarianas costuma ser difícil. Os sintomas associados ao desenvolvimento de câncer ovariano podem mascarar a doença intestinal. Os valores séricos dos marcadores tumorais do antígeno de carboidrato (CA-125) e antígeno carcinoembrionário (CEA) tem um papel importante para a diferenciação entre pacientes com CCR primário com metástase ovariana. Embora difícil, o diagnóstico diferencial deve ser feito precocemente para minimizar as complicações.
