Clinical management in the diagnosis and treatment of odontogenic infections: an integrative literature review

Resumo: Introduction: Odontogenic infections are commonly found in the oral and maxillofacial region. Due to the diversity of the oral microbiota, they are caused by polymicrobial chains, which initially can settle in the dental tissues and evolve rapidly and progressively. Objective: To investigate some of the main odontogenic infections, analyzing their etiology, diagnosis and the most efficient forms of treatment in order to contribute to a better understanding of the clinical picture and its resolution. Methodology: The search strategies were structured from the databases: PubMed and MEDLINE, Google Scholar, SciELO and LILACS using the descriptors: "Focal Dental Infection", "Dental Infection Control", "Ludwig's Angina", "Antibiotic Prophylaxis" and "Pathology, Oral". Studies were found from 2016 to 2023, in Portuguese, English and Spanish languages, excluding those performed on animals, without results or unpublished, and those that did not comprise odontogenic infections and hospitalized patients. Results: For this review 31 articles were collected, 25 of which were included for the development of the study. Discussion: The selected studies confirm the complexity of the symptomatology of odontogenic infections, which is predominantly composed of edema, erythema, pain, fever and trismus. However, with the evolution of the infectious process, there may be formation of acute or chronic abscess, besides the possible need for hospitalization. Early diagnosis, based on clinical and complementary exams, enables treatment, which combines surgical drainage, antibiotic therapy, and removal of the infectious cause. Conclusion: Anatomical knowledge, as well as professional training in prevention, diagnosis and treatment, is essential to mitigate the severity of these infections.

Metodología: Las estrategias de búsqueda fueron estructuradas a partir de las bases de datos: PubMed y MEDLINE, Google Scholar, SciELO y LILACS utilizando los descriptores: "Focal Dental Infection", "Dental Infection Control", "Ludwig's Angina", "Antibiotic Prophylaxis" y "Pathology, Oral". Fueron encontrados estudios de 2016 a 2023, en los idiomas portugués, inglés y español, excluyendo los realizados en animales, sin resultados o no publicados, y los que no comprendían infecciones odontogénicas y pacientes hospitalizados. Resultados: Para esta revisión se recopilaron 31 artículos, 25 de los cuales fueron incluidos para el desarrollo del estudio. Discusión: Los estudios seleccionados confirman la complejidad de la sintomatología de las infecciones odontogénicas, compuesta predominantemente por edema, eritema, dolor, fiebre y trismo. Sin embargo, con la evolución del proceso infeccioso, puede haber formación de absceso agudo o crónico, además de la posible necesidad de hospitalización. El diagnóstico precoz, basado en exámenes clínicos y complementarios, permite el tratamiento, que combina drenaje quirúrgico, antibioticoterapia y eliminación de la causa infecciosa. Conclusiones: Los conocimientos anatómicos, así como la formación profesional en prevención, diagnóstico y tratamiento, son esenciales para mitigar la gravedad de estas infecciones.Introdução: Infecções odontogênicas são comumente encontradas na região oral e maxilofacial. Devido à diversidade da microbiota bucal, são causadas por cadeias polimicrobianas, que inicialmente podem se depositar nos tecidos dentários e evoluir de forma rápida e progressiva. Objetivo: Investigar algumas das principais infecções odontogênicas, analisando sua etiologia, diagnóstico e as formas de tratamento mais eficientes a fim de contribuir para uma melhor compreensão do quadro clínico e sua resolução. Metodologia: As estratégias de busca foram estruturadas a partir das bases de dados: PubMed e MEDLINE, Google Scholar, SciELO e LILACS com uso dos descritores: “Focal Infection dental”, “Infection Control dental”, “Ludwig’s Angina”, “Antibiotic Prophylaxis” e “Pathology, Oral”. Foram encontrados estudos de 2016 a 2023, nos idiomas português, inglês e espanhol, excluindo-se aqueles realizados em animais, sem resultados ou não publicados, e os que não compreendessem infecções odontogênicas e pacientes hospitalizados. Resultados: Para esta revisão foram colhidos 31 artigos, dos quais 25 foram incluídos para desenvolvimento do estudo. Discussão: Os estudos selecionados confirmam a complexidade da sintomatologia das infecções odontogênicas, que é predominantemente composta por edema, eritema, dor, febre e trismo. Porém, com a evolução do processo infeccioso, pode haver formação de abscesso agudo ou crônico, além da possível necessidade de hospitalização. Seu diagnóstico precoce, baseado em exames clínico e complementares, possibilita o tratamento, que combina drenagem cirúrgica, antibioticoterapia, além de remoção da causa infecciosa.
