Epidemiological study of American Tegumentary Leishmaniasis (ATL) in Northeastern Brazil

Resumo: American Cutaneous Leishmaniasis (ATL) is an infectious, non-contagious disease that causes ulcers on the skin and mucous membranes. The disease is caused by protozoa of the genus Leishmania. The disease is transmitted to humans by the bite of infected female sand flies (species of fly). The present study aimed to quantify and analyze the reported cases of ATL in the Northeast. It was a documentary, retrospective, descriptive research with a quantitative approach. The notified cases of ATL were collected from the Informatics Department of the Unified Health System (DATASUS) having the following variables studied: year, sex, type of entry, clinical form, etiological classification and evolution. Between 2014-2018, 23,767 cases of ATL were reported in the Northeast. Analyzing these cases, by year of notification, the following percentage distribution was obtained: 2014 (23%), 2015 (23%), 2016 (16%), 2017 (21%), 2018 (18%). There was a significant difference between females (63%) and males (37%). It was observed that the type of entry per new case (93%) and the cutaneous clinical form of the disease (96%) prevailed. All notified cases were classified as imported (100%). Regarding the evolution of the cases, the cure was presented as the main outcome (59%). The epidemiological study made it possible to know the aspects of ATL in the Northeast. The study also demonstrates the importance of compulsory notification forms as an instrument that allows assessing the epidemiological profile of cases of ATL as well as providing subsidies for structuring control measures that should be incorporated and prioritized to combat the disease.

La enfermedad es causada por protozoos del género Leishmania. La enfermedad se transmite a los humanos por la picadura de moscas de arena hembra infectadas (especies de moscas). El presente estudio tuvo como objetivo cuantificar y analizar los casos reportados de ATL en el noreste. Fue una investigación documental, retrospectiva, descriptiva con un enfoque cuantitativo. Los casos notificados de ATL se recogieron del Departamento de Informática del Sistema Único de Salud (DATASUS) con las siguientes variables estudiadas: año, sexo, tipo de ingreso, forma clínica, clasificación etiológica y evolución. Entre 2014-2018, se reportaron 23,767 casos de ATL en el noreste. Analizando estos casos, por año de notificación, se obtuvo la siguiente distribución porcentual: 2014 (23%), 2015 (23%), 2016 (16%), 2017 (21%), 2018 (18%). Hubo una diferencia significativa entre las mujeres (63%) y los hombres (37%). Se observó que prevalecía el tipo de ingreso por caso nuevo (93%) y la forma clínica cutánea de la enfermedad (96%). Todos los casos notificados se clasificaron como importados (100%). En cuanto a la evolución de los casos, la cura se presentó como el resultado principal (59%). El estudio epidemiológico permitió conocer los aspectos de ATL en el noreste. El estudio también demuestra la importancia de los formularios de notificación obligatoria como un instrumento que permite evaluar el perfil epidemiológico de los casos de LTA, así como proporcionar subsidios para estructurar medidas de control que deben incorporarse y priorizarse para combatir la enfermedad.A Leishmaniose Tegumentar Americana (LTA) é uma doença infecciosa, não contagiosa, que provoca úlceras na pele e mucosas. A doença é causada por protozoários do gênero Leishmania. Ela é transmitida ao ser humano pela picada das fêmeas de flebotomíneos (espécie de mosca) infectadas. O presente estudo teve por objetivo quantificar e analisar os casos notificados de LTA no Nordeste. Tratou-se de uma pesquisa documental, retrospectiva, descritiva com abordagem quantitativa. Os casos notificados de LTA foram coletados a partir do Departamento de Informática do Sistema Único de Saúde (DATASUS) tendo como variáveis estudadas: ano, sexo, tipo de entrada, forma clínica, classificação etiológica e evolução. No intervalo entre 2014-2018 foram notificados 23.767 casos de LTA no Nordeste. Analisando esses casos, por ano de notificação, obteve-se a seguinte distribuição percentual: 2014 (23%), 2015 (23%), 2016 (16%), 2017 (21%), 2018 (18%). Houve diferença significativa entre o sexo feminino (63%) e o masculino (37%). Observou-se que prevaleceu o tipo de entrada por caso novo (93%) e, a forma clínica cutânea da doença (96%). Todos os casos notificados foram classificados como importados (100%). Quanto a evolução dos casos, a cura apresentou-se como o desfecho principal (59%). O estudo epidemiológico possibilitou conhecer os aspectos da LTA no Nordeste.
