Psychoanalysis and multidisciplinary practice: (im) possible consequences of working with autistic children and parents

Resumo: Difficulties are often associated with suffering, but they can also be a motto for reflection and care. In this article we worked on some developments of the research developed over six years in a university hospital in the north of the country, more specifically those that emerged from clinical interventions in the service that welcomes children with autism, severe neurological syndromes, learning problems, hyperactivity, among other diagnoses. As analysts at the hospital, guided by the clinical method, we start from the freudo-lacanian assumptions that support the diagnostic suspension, until it has apprehended, in addition to organic causes, the symbolic plots imbricated with the child's symptom. As a working tool, we use listening from parents and children, transference and the therapeutic relationship as central axes for treatment, as we believe that a hasty diagnosis can precipitate drug prescriptions that can have harmful side effects for child development. We problematize the difficulties in clinical work in institutions which, many times, work against the bet on psychic health with its particularities. It was possible to verify that there is a possibility to sensitize the team to the importance of welcoming what is of the experience, giving some possibility of binding to the subject of the unconscious. Therefore, we must effectively support the possibility of the subject's emergence with the team, who is divided and who carries knowledge that he does not know. Important bet, both in the treatment of the child and his parents, as well as in the multidisciplinary work, in which we continue in the (im) possible exercise of intertextuality.

En este artículo trabajamos algunos desarrollos de la investigación desarrollada a lo largo de seis años en un hospital universitario del norte del país, más específicamente los que surgieron de intervenciones clínicas en el servicio que acoge a niños con autismo, síndromes neurológicos severos, problemas de aprendizaje, etc. hiperactividad, entre otros diagnósticos. Como analistas en el hospital, guiados por el método clínico, partimos de los supuestos freudo-lacanianos que sustentan la suspensión diagnóstica, hasta aprehender, además de las causas orgánicas, las tramas simbólicas imbricadas con el síntoma del niño. Por ello, como herramienta de trabajo, utilizamos la escucha de los padres y del niño, la transferencia y la relación terapéutica como ejes centrales del tratamiento, pues creemos que un diagnóstico apresurado puede precipitar la prescripción de medicamentos que pueden tener efectos nocivos en el desarrollo del niño. Problematizamos las dificultades del trabajo clínico en instituciones que, muchas veces, se oponen a la apuesta por la salud psíquica con sus particularidades. Se pudo constactar que existe la posibilidad de sensibilizar al equipo sobre la importancia de acoger lo que es de la experiencia, dando alguna posibilidad de vinculación al sujeto del inconsciente. Por lo tanto, debemos apoyar de manera efectiva la posibilidad de que el sujeto emerja con el equipo, que está dividido y que porta conocimientos que no conoce. Apuesta importante, tanto en el trato del niño y sus padres, como en el trabajo multidisciplinar, en el que continuamos en el (im) posible ejercicio de la intertextualidad.Dificuldades costumam estar associadas a sofrimento, mas podem também ser mote para reflexão e cuidado.  Nesse artigo trabalhamos alguns desdobramentos da pesquisa desenvolvida ao longo de seis anos em um hospital universitário do norte do país, mais especificamente aqueles que surgiram a partir das intervenções clínicas no serviço que acolhe crianças com autismo, síndromes neurológicas graves, problemas de aprendizagem, hiperatividade, dentre outros diagnósticos. Como analistas no hospital, orientadas pelo método clínico, partimos dos pressupostos freudo-lacanianos que sustentam a suspensão diagnóstica, até que se tenha apreendido, para além das causas orgânicas, as tramas simbólicas imbricadas com o sintoma da criança. Como instrumento de trabalho utilizamos a escuta dos pais e da criança, a transferência e a relação terapêutica como eixos centrais para o tratamento, pois acreditamos que o diagnóstico apressado pode precipitar prescrições medicamentosas cujos efeitos seriam prejudiciais para o desenvolvimento infantil. Problematizamos as dificuldades no trabalho clínico nas instituições as quais, muitas vezes, funcionam na contramão da aposta na saúde psíquica com suas particularidades. Foi possível constatar que há possibilidade de sensibilizar a equipe para a importância de acolher o que é da ordem da experiência, dando alguma possibilidade de amarração para o sujeito do inconsciente. Para tanto, devemos efetivamente sustentar junto à equipe, a possibilidade de emergência do sujeito, este que é dividido e que porta um saber que ele desconhece.
