O COMPOSTO LÁCTEO COMO SUBSTITUTO DO LEITE MATERNO – UM DESAFIO PARA A SAÚDE PÚBLICA BRASILEIRA.

Resumo: Introduction: The growing-up milk is a powdered food product, which follows minimum standards of identity, such as: 51% dairy ingredients and the possibility of adherence of non-dairy components. The product should not be consumed by infants (younger than 6 years) due to the content of sugar and the level of processing, however, aggressive marketing strategies confuse parents and caregivers by the visual similarity between the packaging of growing-up milk, powdered milk and especially, infant formula. The risk derived from the inappropriate consumption of the product by young children is not well elucidated yet. Objective: To verify the impact of growing-up milk consumption on children's nutrition and to evaluate the knowledge of health professionals about this product and the pertinent legislation (Norma Brasileira de Comercialização de Alimentos para Lactentes e Crianças de Primeira Infância, Bicos, Chupetas e Mamadeiras - NBCAL). Methods: This research was approved by the ethics committee for research with human beings at Unioeste under number 39704220.0.0000. 0107/ 2020, conducted in Toledo/Pr and structured in 5 stages: 1) integrative review on the growing-up milk; 2) survey and analysis of nutritional characteristics of the growing-up milk; 3) interview with prescribing professionals (physicians and nutritionists) of breastmilk substitutes, working in the Sistema Único de Saúde (SUS); and interview with pharmacists from the commercial area; 4) monitoring of the sale of the product in pharmacies; 5) proposition of solutions, based on the literature and the results of this research. In the integrative review, 11 articles from 3 different databases were analyzed, within the time limit of 10 years. The interviews were conducted remotely due to the COVID-19 pandemic with the support of an online tool. The approach towards pharmacists occurred in conjunction within the commercial promotion monitoring in pharmacies, where the research was exposed and an electronic contact form was requested in order to send the survey link to the professional. The contact with physicians and nutritionists occurred electronically, through e-mail and social networks, and in some cases, in person. The monitoring of the commercial promotion of the growing-up milk was conducted presentially and based on the NBCAL and Decree n. 9.579/2018. Excel software and descriptive statistics were used for data analysis and interpretation. Results: The growing-up milk sold in Brazil has excessive amounts of protein and added sugars, predominance of saturated fats, as well chemical additives. It was possible to categorize the growing-up milk into three different groups (1, 2 and 3) according to the kind of ingredients featured in each formulation. After statistical analysis, it was concluded that despite the group and the ingredients, the consumption of growing-up milk has a negative impact on children's health, and the younger the age at which growing-up milk is introduced, the bigger the impact on health. The NBCAL monitoring shows that 58% of the pharmaceutical establishments in Toledo violate the legislation in some way, as 91% of follow-up formulas and 96.9% of fluid or powdered milks and growing-up milk were commercially promoted without the mandatory warning phrases. Health professionals have insufficient knowledge of the NBCAL and have difficulty to identify the growing-up milk. Due to the lack of assurance about the potential harm caused by the growing-up milk, adherence to the Precautionary Principle is recommended.

O produto não deveria ser consumido por crianças de primeira infância (menores de 6 anos) pela presença de açúcar e devido ao nível de processamento; entretanto, estratégias agressivas de marketing confundem pais e cuidadores pela semelhança visual entre embalagens de composto lácteo, leite em pó e, principalmente, de fórmula infantil. O risco advindo do consumo inadequado do produto por crianças de primeira infância ainda não é bem esclarecido. Objetivo: Verificar o impacto do consumo de composto lácteo sobre a nutrição infantil e avaliar o conhecimento dos profissionais de saúde sobre este produto e sobre a legislação pertinente (Norma Brasileira de Comercialização de Alimentos para Lactentes e Crianças de Primeira Infância, Bicos, Chupetas e Mamadeiras - NBCAL). Métodos: A pesquisa foi aprovada pelo comitê de ética em pesquisa com seres humanos da Unioeste sob o número 39704220.0.0000.0107/ 2020, aplicada em Toledo/PR e dividida em 5 etapas: 1) revisão integrativa sobre o composto lácteo; 2) levantamento e análise de características nutricionais do composto lácteo; 3) entrevista com profissionais prescritores (médicos e nutricionistas) de substitutos do leite materno, atuantes no Sistema Único de Saúde (SUS); e entrevista com farmacêuticos da área comercial; 4) monitoramento da venda do produto em farmácias; 5) proposição de soluções, baseado na literatura e nos resultados do presente trabalho. Na revisão integrativa foram analisados 11 trabalhos de 3 bases de dados diferentes, dentro do limite de tempo de 10 anos. As entrevistas foram realizadas de forma remota devido à pandemia da COVID-19 com auxílio de ferramenta on-line. O contato com os farmacêuticos ocorreu junto ao monitoramento da promoção comercial nas farmácias, em que foi apresentada a pesquisa e solicitado uma forma de contato eletrônico para enviar o link da pesquisa ao profissional. O contato com médicos e nutricionistas ocorreu de forma eletrônica, por meio de e-mail e redes sociais e, em alguns casos, presencialmente. O monitoramento da promoção comercial do composto lácteo foi realizado de forma presencial e baseado na NBCAL e no Decreto n. 9.579/2018. Utilizou-se o software Excel e estatística descritiva para a análise e interpretação dos dados. Resultados: O composto lácteo vendido no Brasil tem excesso de proteínas e açúcares de adição, predominância de gorduras saturadas, além de aditivos químicos. Foi possível categorizar o composto lácteo em três grupos diferentes (1, 2 e 3) de acordo com o tipo de ingrediente apresentado em cada formulação. Após análises estatísticas, concluiu-se que, independente do grupo e dos ingredientes, o consumo de composto lácteo tem impacto negativo à saúde infantil, sendo quanto menor a idade de introdução do composto lácteo, maior o impacto causado na saúde. O monitoramento da NBCAL mostra que 58% dos estabelecimentos farmacêuticos de Toledo infringem a legislação de alguma forma, sendo que 91% das fórmulas infantis de seguimento para crianças de primeira infância e 96,9% dos leites fluídos ou em pó e composto lácteo, foram promovidos comercialmente sem as frases de advertência obrigatórias. Os profissionais de saúde possuem conhecimento insuficiente sobre a NBCAL e dificuldade para identificar o composto lácteo. Devido à ausência de certeza sobre o potencial dano causado pelo composto lácteo, recomenda-se a adoção do Princípio da Precaução. Conclusão: O composto lácteo é um produto inadequado ao consumo por crianças de primeira infância devido a sua composição. A grande variação de ingredientes e quantidade de macronutrientes representa risco nutricional, sendo possível inferir que consumo a longo prazo é potencialmente danoso, em especial às crianças que tem contato com o composto lácteo antes dos 6 meses de vida. description Introduction: The growing-up milk is a powdered food product, which follows minimum standards of identity, such as: 51% dairy ingredients and the possibility of adherence of non-dairy components. The product should not be consumed by infants (younger than 6 years) due to the content of sugar and the level of processing, however, aggressive marketing strategies confuse parents and caregivers by the visual similarity between the packaging of growing-up milk, powdered milk and especially, infant formula. The risk derived from the inappropriate consumption of the product by young children is not well elucidated yet. Objective: To verify the impact of growing-up milk consumption on children's nutrition and to evaluate the knowledge of health professionals about this product and the pertinent legislation (Norma Brasileira de Comercialização de Alimentos para Lactentes e Crianças de Primeira Infância, Bicos, Chupetas e Mamadeiras - NBCAL). Methods: This research was approved by the ethics committee for research with human beings at Unioeste under number 39704220.0.0000. 0107/ 2020, conducted in Toledo/Pr and structured in 5 stages: 1) integrative review on the growing-up milk; 2) survey and analysis of nutritional characteristics of the growing-up milk; 3) interview with prescribing professionals (physicians and nutritionists) of breastmilk substitutes, working in the Sistema Único de Saúde (SUS); and interview with pharmacists from the commercial area; 4) monitoring of the sale of the product in pharmacies; 5) proposition of solutions, based on the literature and the results of this research. In the integrative review, 11 articles from 3 different databases were analyzed, within the time limit of 10 years. The interviews were conducted remotely due to the COVID-19 pandemic with the support of an online tool. The approach towards pharmacists occurred in conjunction within the commercial promotion monitoring in pharmacies, where the research was exposed and an electronic contact form was requested in order to send the survey link to the professional. The contact with physicians and nutritionists occurred electronically, through e-mail and social networks, and in some cases, in person. The monitoring of the commercial promotion of the growing-up milk was conducted presentially and based on the NBCAL and Decree n. 9.579/2018. Excel software and descriptive statistics were used for data analysis and interpretation. Results: The growing-up milk sold in Brazil has excessive amounts of protein and added sugars, predominance of saturated fats, as well chemical additives. It was possible to categorize the growing-up milk into three different groups (1, 2 and 3) according to the kind of ingredients featured in each formulation. After statistical analysis, it was concluded that despite the group and the ingredients, the consumption of growing-up milk has a negative impact on children's health, and the younger the age at which growing-up milk is introduced, the bigger the impact on health. The NBCAL monitoring shows that 58% of the pharmaceutical establishments in Toledo violate the legislation in some way, as 91% of follow-up formulas and 96.9% of fluid or powdered milks and growing-up milk were commercially promoted without the mandatory warning phrases. Health professionals have insufficient knowledge of the NBCAL and have difficulty to identify the growing-up milk. Due to the lack of assurance about the potential harm caused by the growing-up milk, adherence to the Precautionary Principle is recommended.
