Risk of infection for the cancer patient using totally implanted central venous cateter – Integrative review

Resumo: Background and objectives: Totally implantable central venous catheter has been increasingly used in cancer patients and is capable of minimizing complications resulting from peripheral intravenous therapy, being relevant for patient safety during the infusion of antineoplastic drugs. The aim of this study is to identify the manipulation behavior, to gather and synthesize avaliable evidences in the literature about measures of prevention and control of IRAS related to the use of totally implantable central venous catheter. Contents: An integrative review was carried out in the period from 2009 to 2016 through the databases MEDLINE (Literature Analysis and Retrieval System Online) and LILACS (Latin American and Caribbean Literature in Health Sciences), which resulted in the selection of 10 articles, of which the majority was national (six) and Portuguese (seven).The PICO strategy, which consists of an acrostic: Patient, Intervention, Comparison and Outcomes. Complications, management, maintenance of totally implantable central venous catheter and the relevance of the technoscientific knowledge of the nurse who handled this device were considered, aiming at quality and safety of care. Conclusion: According to the analyzed articles, the duration of the catheter, the complications inherent to the use as obstruction, infection, extravasation, thrombosis and displacement, as well as the handling, maintenance and puncture of the device were the subjects evidenced. For the purposes of intervention by the nurse, it is explicit that if there is a standardization through education and training of the nursing team, it is possible to work towards the prevention of the potential for infection of central venous catheterization totally implanted in cancer patients providing greater safety in the handling of the catheter and preventing the appearance of complications, which may guarantee qualified nursing care and, consequently, a better quality of life for the patients attended.

Contenido: Se realizó una revisión integrativa en el período de 2009 a 2016 a través de las bases de datos, MEDLINE (Literatura Analysis and Retrieval System Online) y LILACS (Literatura Latinoamericana y del Caribe en Ciencias de la Salud) que resultó en la selección de 10 artículos, de los cuales la mayoría eran nacionales (seis) y en portugués (siete). Para el levantamiento en la literatura y selección de la cuestión del estudio, se utilizó la estrategia PICO que consiste en un acróstico: Paciente, Intervención, Comparación y Outcomes / desecado. Fueron abordadas complicaciones, manejo, mantenimiento de catéteres totalmente implantados y la relevancia del conocimiento tecnocientífico del enfermero que manipula ese dispositivo, buscando calidad y seguridad de la asistencia. Conclusión: Según los artículos analizados, el tiempo de permanencia del catéter, las complicaciones inherentes al uso como obstrucción, infección, extravasación, trombosis y desplazamiento, así como el manejo / manoseo, mantenimiento y punción del dispositivo fueron los asuntos evidenciados. Para los efectos de intervención por parte de la enfermera, queda explícito que habiendo una estandarización por medio de educación y capacitación del equipo de enfermería, es posible trabajar en pro de la prevención del potencial para infección del cateterismo venoso central totalmente implantado en pacientes oncológicos propiciando mayor seguridad en el manejo del catéter y previniendo el surgimiento de complicaciones, lo que podrá garantizar asistencia de enfermería calificada y, consecuentemente, mejor calidad de vida a los pacientes atendidos.Justificativa e objetivos: O cateter totalmente implantado tem sido cada vez mais utilizado em pacientes oncológicos e é capaz de minimizar complicações decorrentes da terapia intravenosa periférica, sendo relevante para segurança do paciente na infusão de drogas antineoplásicas. Este estudo teve como objetivo identificar as condutas de manipulação, reunir e sintetizar as evidências disponíveis na literatura sobre medidas de prevenção e controle de IRAS relacionadas ao uso de catéteres totalmente implantados. Conteúdo: Realizou-se uma revisão integrativa no período de 2009 a 2016 por meio das bases de dados, MEDLINE (Literature Analysis and Retrieval System Online) e LILACS (Literatura Latino-Americana e do Caribe em Ciências da Saúde) que resultou na seleção de 10 artigos, dos quais a maioria era nacional (seis) e em português (sete). Conclusão: Segundo os artigos analisados, o tempo de permanência do cateter, às complicações inerentes ao uso como obstrução, infecção, extravasamento, trombose e deslocamento, bem como o manejo/ manuseio, manutenção e punção do dispositivo foram os assuntos evidenciados.
