Patients with gummy smile, mostly are not satisfied with the facial aesthetics. With the improvement of knowledge, skill and technique the dentist is able to change the positions of the gingival margin and incisal edges, among others for achieving a harmonious and aesthetic smile. The periodontal surgery allows you to change the position of the gingival margin in order to make the relationship gum/tooth more proportional. In some cases, to achieve better cosmetic surgical treatment may be accompanied by restoration treatment. The ceramic is presented as one of the best choices of restorative material, because it has excellent aesthetics, biocompatibility, color stability and is capable of playing quite like the tooth structure. The present study aims to describe the surgical technique of a periodontal treatment and making crows and ceramic facets in cosmetic and functional rehabilitation. There will be a case report of a patient with poor gingival smile and aesthetics, which will be performed periodontal surgery and ceramic facets of the elements 13, 12, 11, 21, 22 and total crown in the element 23
