Análise da resistência ao destorque de parafusos protéticos e parafusos de pilar em reabilitações totais fixas conceito all-on-four

Resumo: The loosening of the prosthetic screws and screw the conical abutments are presented as a risk factor for prosthetic implant. The aim of this study was to evaluate the value of detorque screws, prosthetic and abutment screw tapered straight. In 18 patients were analyzed 10 and 8 jaws jaws rehabilitated by the All-on-four. The torqueamento 35 N.cm was performed in 72 tapered abutment screws and 15 in 72 N.cm prosthetic screws and torque. After 24 months of prosthetic facility, under the concept for immediate loading, held the destorqueamento the prosthetic screws and conical pillar. In tapered abutment screws in the jaw, there was an average value of 26.1 destoque N.cm pillars distal and 30.9 N.cm in abutments. In the maxilla, the average detorque for tapered abutment screws was 29.7 and 31.5 N.cm in distal N.cm in abutments. In prosthetic screws showed a mean of 11.3 detorque mandibular N.cm in distal screws and bolts in 12.6 N.cm intermediaries. Prosthetic screws torqued in the maxilla showed a mean value of 11.8 detorque N.cm in distal screws and 12.3 N.cm in prosthetic screws intermediaries. Conclusion: Within the limits of this study, we conclude that the screw loosening occurred in both the abutments as in prosthetic screws. The lowest value was found in the detorque prosthetic screws and distal abutment screws. From the results it is suggested that one should perform periodic retightening.

O objetivo deste estudo foi avaliar o valor do destorque dos parafusos, protético e parafuso de pilar cônico reto. Em 18 pacientes foram analisadas 10 maxilas e 8 mandíbulas reabilitadas pelo conceito All on four. O torqueamento de 35 N.cm foi realizado em 72 parafusos de pilar cônico e de 15 N.cm em 72 parafusos protéticos e torque. Após 24 meses da instalação protética, sob o conceito em carga imediata, realizou-se o destorqueamento dos parafusos protéticos e de pilar cônico. Em parafusos de pilares cônico, em mandíbula, observou-se uma média do valor de destoque de 26,1 N.cm em pilares distais e de 30,9 N.cm em pilares intermediários. Em maxila, a média do destorque para parafusos de pilar cônico foi 29,7 N.cm em distais e 31,5 N.cm em pilares intermediários. Em parafusos protéticos observou-se uma média do destorque em mandíbula de 11,3 N.cm em parafusos distais e 12,6 N.cm em parafusos intermediários. Parafusos protéticos torqueados em maxila apresentaram um valor médio de destorque de 11,8 N.cm em parafusos distais e 12,3 N.cm em parafusos protéticos intermediários. Conclusão: Dentro dos limites do presente estudo, podemos concluir que o afrouxamento do parafuso ocorreu tanto nos pilares protéticos como nos parafusos protéticos. Foi encontrado o menor valor do destorque em parafusos protéticos e parafusos de pilares distais.
