The changes in the State-society-economy relation have been demanding a restructuring of the economic and political organization of the countríes, with side effects on the role of the State and consequently on the health sectoro In Brazil, in the 90's, the State reform process is based on a triple diagnosis:an increasing fiscal crisis, an exhaustion of the protectionist forms of intervention in the economy and a public management considered excessively bureaucratic and inefficient. The construction of the capacity of the State, established mainly in the efficiency concept, is assumed as the central objective of the reform of the Brazilian State and of its apparatus. This paper reviews this process of reform, focusing one of its key concepts - efficiency - and bringing it to the context of the public health sector.
