The sectorial reform of the Brazilian State in the 90s: efficiency and privatization? A study in a university public hospital

Resumo: Healthcare organizations have been under pressure to improve their efficiency and effectiveness. The first demands for change to take place in hospitals were in the methods for financing and paying healthcare procedures and in the introduction of new regulation agents. On the other hand, the sectorial reform process of the Brazilian State has intensified amidst a fiscal crisis and a paradigm change in the new world order. This article discusses how this reform environment, with budget constraints and new rules, induced adaptations and institutional changes in the public-private mix of a university public hospital. The article presents a case study carried out in the Federal University of Minas Gerais Clinics Hospital (HC/UFMG), analyzing production indicators covering the 1996-2000 period and how workers and management perceived the opening of the hospital for private health insurance plans as an alternative financing source. The study showed the importance of this source of financing for the hospital’s economy, as well as contradiction and resistance, by management and workers, against private financing as a survival means.

As organizações de saúde vêm experimentando pressões para aumentar a eficiência e a efetividade. As primeiras pressões por mudanças nos hospitais foram nos métodos de financiamento e remuneração das ações de saúde e na introdução de novos agentes de regulação. Por sua vez, acentua-se o processo de reforma setorial do Estado em meio a uma crise fiscal e a uma mudança de paradigma na nova ordem mundial. Este artigo discute como esse ambiente de reforma, com restrições orçamentárias e novas regras, induziu adaptações e mudanças institucionais do mix público-privado num hospital público universitário. O artigo apresenta um estudo de caso do Hospital das Clínicas da Universidade Federal de Minas Gerais, analisando indicadores de produção entre 1996 e 2000 e a percepção dos trabalhadores e da gerência quanto à abertura do hospital para planos e seguros privados de saúde como alternativa de financiamento. Para análise da percepção utilizaram-se entrevistas semi-estruturadas com o corpo diretivo e grupos focais com os trabalhadores.
