Challenges in the training of mid-level workers for the Unified Health System in the state of Maranhão, Brazil

Resumo: | Historically, the training of mid-level health workers has been provided sporadically, with a focus on know-how. Within the context of the Brazilian Sanitary Reform, initiatives such as the ‘Large-Scale Training Project’ boosted the transition to an understanding of the critical-reflective technical work, and required a new model of training for basic- and mid-level workers. The Mid-Level Professional Training Program is the most recent large-scope initiative to qualify the work in the Unified Health System. We analyzed the profile, the work placement and the perception regarding the training process of this Training Program in the state of Maranhão, Brazil, of 404 graduates from the Technical School of the Unified Health System in Maranhão. We identified that: the socioeconomic profile of the graduates seems to mirror social inequalities; the teaching-learning process was in line with the guidelines of the Unified Health System, with pedagogical contradictions to be considered; and that there is a difficulty in retaining the trained professionals, which is expressed by the loss of ties, and the migration to private health services and to other fields not related to health. The qualification of professionals is complex, requiring actions in the field of political-pedagogical projects and work management projects, which go beyond the extension of the offer of enrollment spots in professionalization courses. |

En el contexto de la Reforma Sanitaria Brasileña, iniciativas como el ‘Proyecto de Formación en Gran Escala’ impulsaron la transición para una comprensión del trabajo técnico crítico-reflexivo, y demandaron un nuevo modelo de formación de trabajadores de los niveles básico y medio. El Programa de Formación de Profesionales de Nivel Medio para la Salud es la más nueva iniciativa de amplio alcance para cualificar el trabajo en el Sistema Único de Salud. Analizamos el perfil, la inserción laboral y la percepción sobre el proceso formativo de ese Programa de Formación en el estado de Maranhão de 404 egresos de la Escuela Técnica del Sistema Único de Salud de Maranhão. Identificamos que: el perfil socioeconómico de los egresos parece reproducir las desigualdades sociales; el proceso de enseñanza-aprendizaje estaba alineado a las directrices del Sistema Único de Salud, con contradicciones pedagógicas a considerar; y que hay una dificultad en la retención de los profesionales formados, expresa por la pérdida de vínculos, y por la migración para los servicios privados de salud y para otras áreas ajenas a la salud. La capacitación de trabajadores es compleja, y demanda acciones articuladas en el campo de los proyectos político-pedagógicos y de la gestión del trabajo que superan la ampliación de la oferta de plazas en cursos de profesionalización.Historicamente, a formação de trabalhadores de nível médio para a saúde ocorreu pontualmente, com enfoque no saber-fazer. No contexto da Reforma Sanitária, iniciativas como o ‘Projeto de Formação em Larga Escala’ impulsionaram a transição para uma compreensão do trabalho técnico crítico-reflexivo, e demandaram um novo modelo de formação de trabalhadores do nível fundamental e médio. O Programa de Formação de Profissionais de Nível Médio para a Saúde é a mais recente iniciativa de amplo alcance para qualificar o trabalho no Sistema Único de Saúde. Analisou-se o perfil, a inserção no trabalho e a percepção acerca do processo formativo desse Programa de Formação no Maranhão de 404 egressos da Escola Técnica do Sistema Único de Saúde do Maranhão. Identificou-se que: o perfil socioeconômico de egressos parece reproduzir as desigualdades sociais; o processo ensino-aprendizagem foi alinhado às diretrizes do Sistema Único de Saúde, com contradições pedagógicas a considerar; e há dificuldade na fixação dos profissionais formados, expressa pela perda de vínculos, migração para os serviços privados de saúde e para outras áreas alheias à saúde.
